摘要
目的探讨奥曲肽对经皮经肝穿刺胆道介入治疗后高淀粉酶血症和急性胰腺炎的预防作用。方法纳入接受胆道介入治疗的90例患者,随机分为预防组41例和对照组49例,分别于术前1h静脉滴注奥曲肽0.30mg+生理盐水250ml和生理盐水250ml,持续6h;于术前、术后6h和24h检测血清淀粉酶水平,观察相关症状及体征。结果两组术前血清淀粉酶水平差异无统计学意义(P>0.05);预防组术后6h和24h淀粉酶水平明显低于对照组(P均<0.01);预防组高淀粉酶血症和急性胰腺炎发生率分别为17.07%和2.44%,对照组分别为26.53%和12.24%,预防组均低于对照组(P均<0.05)。结论术前预防性应用奥曲肽可降低经皮经肝穿刺胆道介入治疗术后高淀粉酶血症及急性胰腺炎的发生率。
Objective To explore the preventive effect of octreotide on hyperamylasemia and acute pancreatitis after percutaneous transhepatic biliary interventional therapies.Methods Totally 90 patients who underwent biliary interventional therapy were randomly divided into prevention group（n=41） and control group（n=49）.Octreotide 0.3 mg＋normal saline 250 ml for prevention group or normal saline 250 ml for control group were administrated via venous 1 h before operation and lasted for 6 h.The concentration of serum amylase before operation and 6 h,24 h after operation were measured.The abdominal symptoms and signs were observed.Results There was no difference in the concentration of serum amylase between the two groups before operation（P0.05）.The level of serum amylase in prevention group was significantly lower than that in control group 6 h and 24 h after operation（both P0.01）,and the incidence of hyperamylasemia and acute pancreatitis in prevention group were both lower than those in control group（17.07% vs 26.53%,2.44% vs 12.24%,both P0.05）.Conclusion Preoperative preventive administration of octreotide can reduce the incidence of hyperamylasemia and acute pancreatitis after percutaneous transhepatic biliary interventional therapies.
