摘要
目的探讨低透析液流量、低血流量、长时透析对心血管系统的影响。方法选取2009年5月至2011年5月武汉大学人民医院肾内科诊治的40例血液透析患者,按照随机配对原则将患者分为试验组和对照组,试验组20例,每次透析时间6~8 h,透析液流量0.3 L/min,血流量0.15~0.2 L/min,透析频率3次/周;对照组20例,每次透析时间4 h,透析液流量0.5 L/min,血流量0.25 L/min,透析频率3次/周。比较两组患者心血管事件发生率,超声心动图监测治疗前后心功能指标的变化。结果试验组患者高血压、低血压、心力衰竭、心律失常等心血管事件发生率较对照组显著降低（P〈0.05）;试验组透析后心脏收缩功能指标：每分搏出量、左心射血分数和左心室质量指数以及舒张功能指标均较治疗前明显好转,且治疗后的效果显著高于对照组（P〈0.05）。结论低透析液流量、低血流量、长时透析能够显著改善高血压,改善心功能,逆转左心室重构,值得临床进一步研究推广。
Objective To investigate the influence of the long-time low dialysate flow rate,low blood flow dialysis on the cardiovascular system.Methods Forty cases of dialysis patients from Department of Nephrology of Renmin Hospital of Wuhan University during May 2009 and May 2011 were selected and randomly divided into the trial group and the control group.20 cases of the trial group： dialysis time was 6-8 h every time,dialysate flow rate was 0.3 L / min,blood flow was 0.15-0.2 L / min,dialysis frequency was 3 times / week;20 cases of control group： dialysis time was 4 h every time,dialysate flow rate was 0.5 L / min,blood flow was 0,25 L / min,dialysis frequency was 3 times / week.The incidence of cardiovascular events of two groups was compared,the cardiac index changes was observed by ultrasonic cardiogram before and after treatment.Results The incidence of cardiovascular events such as hypertension,hypotension,heart failure,arrhythmia of the trial group was lower than the control group,the difference was statistically significant（P 0.05）;cardiac systolic function indices including every minute stroke volume,left ventricular ejection fraction and left ventricular mass index,and diastolic function index of the trial group were better than before treatment and the control group after treatment,the differences was statistically significant（P 0.05）.Conclusion Long-time low dialysate flow rate,low blood flow and dialysis can improve hypertension significantly,improve heart function,and reverse left ventricular remodeling,which is worthy of further clinical research and promotion.
