摘要
目的研究双氯芬酸钠滴眼液促进糖尿病视网膜病变(DR)患者瞳孔散大的合理用药时间。方法双眼病变程度相同的DR患者50例100眼,同一患者两眼随机分为试验组和对照组。试验组于散瞳前24 h使用双氯芬酸钠滴眼液,早、中、晚各一次,每次1滴;对照组于散瞳前15 min使用双氯芬酸钠滴眼液三次,每次1滴。两组同时应用复方托吡卡胺滴眼液散瞳,15 min内点眼三次,每次1滴。检测用药前、散瞳前、散瞳后两组的平均瞳孔面积,并观察不良反应。结果用药前两组平均瞳孔面积无显著差异(P>0.05),散瞳前试验组平均瞳孔面积为(5.67±1.80)mm2,大于对照组[(5.55±1.64)mm2],但组间比较无显著差异(P>0.05)。散瞳后试验组和对照组平均瞳孔面积分别为(41.97±10.23)和(37.35±9.99)mm2,差异显著(P<0.05)。两组均无严重不良反应发生。结论双氯芬酸钠滴眼液散瞳前24 h应用比与散瞳药物同时应用更能有效促进DR患者瞳孔的散大。
AIM To study the optimal administrative timing of diclofenac sodium for improving pupillary dilation in patients with diabetic retinopathy (DR). METHODS A total of 50 DR patients (100 eyes) with the comparable lesions in both eyes were enrolled. One side of eye randomly received the diclofenac sodium eye drops at 24 h before mydriasis as trial group (three times in 24 h, at 24 h, 20 h and 12 h before mydriasis respectively, one drop/time) or 15 rain before mydriasis as control group (three times in 15 min, interval less than lmin, one drop/time), respectively. All patients received the tropicamide phenylephrine eye drops at the same time to dilate the pupil (three times in 15 min, one drop/time). The average pupil areas were measured before diclofenac sodium treatment, before and after tropicamide phenylephrine treatment and the adverse reactions were observed. RESULTS There were no significant differences in pupil areas before diclofenac sodium treatment between the two groups. Similarly, the average pupil areas before tropicamide phenylephrine treatment in trial group was comparable to control group ( (5.67 ± 1.80) mm^2 vs. (5.55 ± 1.64) mm^2, P 〉 0.05). The pupil areas after tropieamide phenylephrine treatment in the trial group were significantly larger than that in control group ((41.97 ± 10.23) mm^2 vs. (37.35 ± 9.99) mm^2, P 〈 0.05). No serious adverse reactions were recorded in two groups. CONCLUSION The administration of the diclofenae sodium eye drops at 24 h before tropieamide phenylephrine treatment is more effective in pupillary dilation in patients with DR compared with that at 15 min before mydriatics.
