摘要
目的:探讨矫正外伤后歪鼻畸形伴鼻中隔偏曲的鼻功能重建方法,并评价其手术疗效。方法:①鼻内进路组:12例C型歪鼻者,采用鼻中隔半贯穿切口加双侧软骨间切口,使用鼻内镜操作;②鼻外进路组:5例S型和10例侧斜型歪鼻者,采用鼻小柱倒V形切口加双侧软骨下切口。矫正鼻中隔的同时,用自体鼻中隔软骨(骨)移植物进行鼻整形术。结果:2组患者术后均满意效果,随访均无症状复发和并发症等,术前、术后鼻阻力比较差异均有统计学意义(P<0.01),2组术前比较、术后比较、术前术后鼻阻力差值(d=术前-术后)比较差异均无统计学意义(P>0.05)。结论:自体鼻中隔软骨和骨是鼻美容整形术最安全且效果好的充填材料;同期鼻整形和鼻中隔矫正术可重建鼻功能并兼顾美观。
Objective：To investigate the nasal functional reconstructive methods for post-traumatic external nasal deformity and nasal septal deviation,and to evaluate the efficacy of surgical treatment.Method：① Twelve patients with C-type external nasal deviation were chosed undertaking L-shaped incision of the interface of left nasal septal skin and mucosa as well as bilateral inter-cartilaginous incision using endoscopy which named close group;②5 patients with S-type and 10 patients with side-skew-type external nasal deviation patients were chosed undertaking trans-columella inversed-V incision as well as bilateral infra-cartilaginous incision viewing directly which named open group.Both of the groups undertook nasal septal reconstruction surgery and rhinoplasty with autologous nasal septal cartilage grafts.Result：During the follow-up period,both groups were satisfied with the functional and aesthetic outcome,while with no recurrence of symptoms and complications.No significant differences were found between the two groups.Conclusion：Autologous nasal septal cartilage and bone are the best filling grafts for cosmetic rhinoplasty.Rhinoplasty with simultaneous nasal septum surgery can reconstruct nasal function and appearance.
