摘要
目的探讨早期持续气道正压通气(CPAP)对于小儿重症肺炎并呼衰中的治疗效果,评价其临床应用价值。方法对于2010年10月-2011年12月在医院进行治疗的68例小儿重症肺炎合并呼衰患者,随机分为两组,观察组患者采用CPAP治疗,对照组患者采用鼻道管吸氧,对两组患者的有效率、呼吸改善、气促、紫绀、血气情况及住院时间等进行比较。结果观察组患者的有效率高于对照组,两组间差异具有统计学意义(P<0.05);观察组患者的PaO2及SaO2水平明显高于对照组患者,两组间差异具有统计学意义(P<0.05);观察组患者的缺氧、改善呼吸、住院时间等均好于对照组,两组间差异具有统计学意义(P<0.05)。结论早期持续CPAP对于小儿重症肺炎并呼衰中的治疗效果较好,不仅能够改善患者的缺氧及呼吸情况,同时能够缩短患者的住院时间,具有很好的临床应用价值。
Objective To study the clinical curative of early continuous positive airway pressure（CPAP） on pediatric severe pneumonia combined with respiratory failure,to evaluate its clinical application value.Methods Total 68 cases of severe pneumonia combined with respiratory failure from October 2010 to December 2011 in our hospital were randomly divided into two groups,the observation group were treated with CPAP,and the control group were treated by nasal oxygen pipeline.The effective rate,ventilatory function improvement,shortness of breath,cyanosis,blood gas analysis and length of hospitalization were compared between the two groups.Results The effective rate in the observation group was higher than those in the control group,the difference between the two groups was statistical significat（P〈0.05）.The levels of PaO2 and SaO2 in the observation group were obviously higher than those in the control group,the difference was statistical significat（P〈0.05）.The improvement in anoxia,respiratory and length of hospital stay was better as compared to the control group,the difference between the two groups have statistical significance（P〈0.05）.Conclusion The early CPAP for pediatric severe pneumonia combined with respiratory failure is effective,it can not only improve the anoxia and breathing,but also shorten the length of hospital stay,and the clinical value is exactly.
