摘要
目的分析医院胃肠道肿瘤患者围手术期临床诊治的相关资料,找出发生医院感染的原因,并制定相应的感染控制措施。方法将2009年1月-2013年1月在医院接受胃肠道肿瘤手术并在围手术期发生感染的90例患者作为感染组,同期的314例无感染患者作为对照组,找出导致胃肠道肿瘤围手术期临床诊治中发生感染的危险因素,并对其进行非条件logistic回归分析,找出独立危险因素,并通过查阅文献和临床经验找出控制感染的相关措施。结果胃肠道肿瘤围手术期临床诊治患者发生医院感染率为22.28%,经logistic回归分析显示,患有基础疾病(β=0.850,OR=5.123)、感染前有放化疗史(β=1.114,OR=6.517)、低蛋白血症(β=0.997,OR=5.835)、住院天数≥7d(β=0.907,OR=5.382)是导致胃肠道肿瘤围手术期临床诊治中感染发生的危险因素。结论胃肠道肿瘤围手术期临床诊治中感染率较高,易引发感染的危险因素较多,在临床工作中,应注意对危险因素的重视并合理预防,确保患者就医安全。
OBJECTIVE To analyze the relevant information of clinical diagnosis and treatment of patients with gastrointestinal tumor during perioperative period and explore the causes for the infections in the clinical diagnosis and treatment during perioperative period so as to put forward corresponding control measures.METHODS A total of 90 patients,who were treated with the operation of gastrointestinal tumor in the hospital from Jan 2009 to Jan 2013 and were infected during perioperative period,were assigned as the infection group,while 314patients without infections in the same period were assigned as the control group.The risk factors for the infections in clinical diagnosis and treatment of gastrointestinal tumor during perioperative period were found out,then the non-conditional logistic regression method was performed to find out the independent risk factors and put forward the control measures by the review of literature and the clinical experience.RESULTS The incidence of infections in the clinical diagnosis and treatment of gastrointestinal tumor during perioperative period was 22.28%.The result of logistic regression analysis showed that suffering from underlying diseases（β=0.850,OR=5.123）,the history of chemotherapy before infection（β=1.114,OR=6.517）,hypoproteinemia（β=0.997,OR=5.835）,and the length of hospital stay more than 7days（β=0.907,OR=5.382）were the risk factors for the infections in clinical diagnosis and treatment of gastrointestinal tumor during perioperative period.Nursing aimed at the easy cause of infection was the effective measure to reduce infection.CONCLUSIONThere are many risk factors for the infections in the clinical diagnosis and treatment of gastrointestinal tumor during perioperative period,it is necessary for the hospital to focus on the reasonable prevention targeting to the risk factors so as to ensure the safety of treatment.
