摘要
目的研究胰岛素泵皮下连续注射法(CSII)和多次皮下注射法(MSII)对糖尿病合并菌阳肺结核疗效的影响,比较2种注射方式的疗效及安全性。方法 62例糖尿病合并初治菌阳肺结核患者随机分为CSII组和MSII组,检测两组患者8个时点血糖,记录血糖达标时间、胰岛素用量、体质量增加和低血糖发生率;观察两组患者抗结核治疗后肺部病灶变化及痰菌阴转情况。结果 CSII组血糖达标时间、胰岛素用量及低血糖的发生率明显少于MSII组,两组比较差异有统计学意义(P<0.05);治疗4周后两组体质量较治疗前均有所增加,但两组比较无统计学意义(P>0.05)。两组患者治疗3个月时肺结核病灶显吸率、空洞闭合率和痰菌阴转率差异均有统计学意义(P<0.05);治疗6个月时CSII组空洞闭合率较对照组相比差异有统计学意义(P<0.05),病灶显吸率和痰菌阴转率较对照组略有提高,但差异无统计学意义(P>0.05)。结论胰岛素泵能快速使血糖达标,减少低血糖的发生率,提高抗结核治疗效果,尤其是早期治疗效果。
【Objective】 The effect of continuous subcutaneous injection by insulin pump and multiple insulin injection in treatment of diabetes mellitus complicated with pulmonary tuberculosis with smear and culture positive is studied with the purpose of comparing their curative effect and security.【Methods】 62 patients of diabetes mellitus complicated with pulmonary tuberculosis were divided into two groups,that is,CSII group and MSII group.The blood sugar was measured for the two groups 8 times a day,period of blood glucose up to scratch,amount of the use of insulin,weight gain and the rate of low blood sugar being recorded.;Changes in pulmonary lesions and sputum conversion condition of the two groups are observed after anti-tuberculosis treatment.【Results】 The curative effect for the two groups differs evidently with each other(P〈 0.05),the period of blood glucose up to scratch,insulin dosage,and the rate of low blood sugar of CSII group being much less that of CSII group.The weight of the patients of the two groups both increases after 3-month treatment.However,there was no statistically significant difference between the two groups(P〈 0.05).Significant difference was observed on the rate of marked tuberculosis foci absorption(P〈 0.05),pulmonary cavity closure and sputum conversion for the two groups after the patients being treated over 3 months.After 6 months,rate of pulmonary cavity closure of CSII group differs evidently with control group(P〈 0.05),and the curative effect on the rate of marked tuberculosis foci absorption and the rate of sputum conversion for CSII group is better than that for Control group,but there was no significant difference between the two groups(P〈 0.05).【Conclusion】 Insulin pump could effectively control the blood glucose to the target more quickly,depress the rate of low blood sugar,and the curative effect for anti-tuberculosis treatment was improved remarkably,especially at early stage.
