摘要
目的探讨十二指肠损伤的简化手术方式及其效果。方法回顾性分析24例十二指肠损伤患者的临床资料,4例患者行十二指肠单纯修补+双套管负压引流术,16例患者行十二指肠单纯修补+空肠逆行置管减压+VSD负压引流术,4例患者行十二指肠端端吻合+空肠逆行置管减压+VSD负压引流术,术后引流管生理盐水持续冲洗+负压吸引。结果全组无死亡病例,24例患者腹部手术时间60～150min,平均(105.7±24.38)min。术后发生十二指肠瘘4例,胰瘘2例,均经负压封闭引流后自行闭合。结论十二指肠损伤简化手术方式能缩短手术时间,取得良好效果,符合损伤控制原则,适合临床应用。
Objective To investigate the simplified surgical approach to duodenal injury and its clinical efficacy.Methods A retrospective analysis was performed in 24 patients with duodenal injury,including 4 patients who underwent simple duodenal repair and double cannula negative pressure drainage,16 patients who underwent simple duodenal repair,retrograde jejunal decompression and vacuum sealing drainage（VSD）,and 4 patients who underwent duodenal anastomosis,retrograde jejunal decompression and VSD.In addition,all patients received continuous postoperative catheter irrigation with saline and vacuum suction.Results No death occurred in all patients.The average abdominal surgery time was（105.7±24.38） minutes（range,60-150 minutes）.After operation,4 patients had duodenal fistula and 2 patients had pancreatic fistula.All of them were healed by VSD.Conclusion Simplified surgical approach to duodenal injury shortens operation time,produces good results,and is consistent with principle of damage control.Therefore,it is suitable for clinical application.
