摘要
目的比较甲钴胺和硫辛酸治疗糖尿病周围神经病变(DPN)的疗效。方法选取2011年1月至2012年6月由江滨医院收治的DPN患者50例作为研究对象,按照随机数字表法将其分为甲钴胺组(25例)和硫酸锌组(25例),两组患者均给予基础治疗方案,在此基础上甲钴胺组加用甲钴胺1000μg/d;硫酸锌组加用硫酸锌600 mg/d。治疗2周后比较两组总体症状评分(TSS)、多伦多神经病变评分系统(TCSS)评分、阳性症状视觉模拟评分(VAS)以及阴性症状临床改善情况。结果治疗后,两组患者的TSS、TCSS评分均较治疗前显著降低(P<0.01),而两组间治疗前后比较差异均无统计学意义(P>0.05)。两组阳性症状VAS均较治疗前显著改善(P<0.01);其中硫辛酸组烧灼VAS评分优于甲钴胺组(P<0.05)。甲钴胺组在痛觉异常点个数、压力觉异常点个数、震动感觉阈值上较治疗前有显著改善(P<0.05);硫辛酸组在痛觉异常点个数,震动感觉阈值上较治疗前有显著改善(P<0.05)。结论甲钴胺和硫辛酸均可短时间内改善DPN患者的阳性症状和部分阴性症状,但改善程度有所差异,临床上可根据患者实际情况选择药物。
Objective To compare the efficacy of mecobalamin and lipoic acid in treatment of diabetes peripheral neuropathy.Methods A total of 50 patients with diabetes peripheral neuropathy in Jiangbin Hospital of Guangxi Zhuang Autonomous Region from Jan.2011 to Jun.2012 were selected and then randomly divided into two groups.All the patients were given basic treatment,mecobalamin group（25 cases） was added with mecobalamin 1000 μg / d,and lipoic acid group（25 case） was added with lipoic acid 600 mg / d.TCSS and TSS score,VAS score and negative symptoms ameliorated status of the two groups after two weeks of treatment were compared.Results After treatment,TCSS and TSS score of the two groups were significantly decreased（P〈0.01）,while there was no statistical significance between the two groups（P〈0.05）.VAS score of positive symptoms of the two group simproved significantly compared with before treatment（P〈0.01）,among which the burning VAS of the lipoic acid group was better than mecobalamin group（P〈0.05）.In mecobalamin group,after treatment,allodynia point number,pressure anormaly points and vibratory sensation threshold level improved significantly（P〈0.05）.In lipoic acid group,after treatment allodynia point number and vibration perception threshold level improved significantly（P〈0.05）.Conclusion Mecobalamin and lipoic acid can significantly improve positive and part of negative symptoms of diabetic perineuropathy,with different improvement degrees,therefore the drug should be selected according to the actual situation of patients in clinic.
