摘要
目的通过检测溃疡性结肠炎(UC)结肠上皮Toll样受体4(TLR4)、环氧化酶-2(COX-2)、前列腺素受体EP2的表达及相互关系的研究,为UC的临床治疗及癌变预防提供新的思路。方法选取33例活动期UC、11例缓解期UC及23例正常人作为对照进行前瞻性研究,进行免疫组化检测每例结肠上皮的TLR4、COX-2、EP2的表达。结果 TLR4、COX-2、EP2在活动期UC患者结肠上皮和炎症细胞中的染色积分分别是8.76±2.829、7.45±3.063、7.82±2.417,在缓解期UC组的染色积分分别是3.73±1.348、3.00±1.183、3.64±1.804,在正常对照组的染色积分分别是4.91±1.782、2.61±1.196、2.65±1.152,活动期UC患者结肠上皮和炎症细胞中的TLR4、COX-2、EP2的表达均高于UC缓解期及正常结肠黏膜(P<0.01),三者之间表达呈正相关;TLR4在活动期UC患者病理I级、Ⅱ级、Ⅲ级的染色积分分别是6.76±2.016、10.36±2.063、12.00±0.000,EP2在活动期UC组病理Ⅰ级、Ⅱ级、Ⅲ级的染色积分分别是6.71±2.519、8.73±1.794、9.60±1.342,活动期UC患者的TLR4、EP2的表达与病理分级呈正相关(P<0.01),COX-2在活动期UC组病理I级、Ⅱ级、Ⅲ级的染色积分分别是6.76±3.212、8.18±2.136、8.20±4.266,COX-2的表达与病理分级无明显相关(P>0.05)。结论 TLR4、COX-2、EP2在活动期UC患者中的表达显著上调,且三者之间呈正相关,提示TLR4及EP2可作为UC治疗及其相关性结直肠癌预防的新靶标。
Objective To provide the new thought to the treatment of ulcerative colitis and cancer chemoprevention by detecting the expressions of TLR4 and COX-2, EP2 in epithelium of UC and studing the relationships among them. Methods Thirty-three patients with active UC, eleven patients with remission of UC and twenty-three normal control subjects were recruited into this trial, the expressions of TLR4, COX-2 and EP2 in colonic tissues of the patients with active UC, remission of UC and normal subject controls were evaluated by immunohistochemistry. Results The dyeing integral of TLR4, COX-2, EP2 in colonic epithelium and in flammatory cells in the patients with active UC are respectively 8.76 ± 2. 829,7.45 ±3. 063,7.82 ±2. 417 ,and those in the remission of UC group are respectively 3.73 _± 1. 348,3.00_± 1. 183, 3.64 ± 1. 804 ,those in the normal control group are respectively 4.91 ± 1. 782,2.61 ± 1. 196,2.65 ± 1. 152. The expressions of TLR4, COX-2 and EP2 protein in colonic epithelium and in flammatory cells in the patients with active UC were higher than those in the patients with remission of UC and normal controls (P 〈 0.01 ). Also there was a direct positive correlation among the expressions of TLR4, COX-2 and EP2. The dyeing integral of TLR4 in activite UC patients pathologic grade Ⅰ, Ⅱ ,Ⅲare respectively 6.76 ± 2. 016,10.36 ± 2. 063,12.00 ± 0. 000, those of EP2 are respectively 6.71 ± 2.519,8.73 ± 1. 794,9.60 ± 1. 342. In the patients with active UC, the expressions of TLR4 and EP2 were closely related with pathological grade ( P 〈 0.01 ) ; However the dyeing integral of cox-2 in activite UC patients pathologic grade Ⅰ, Ⅱ ,Ⅲ are respectively 6.76 ± 3. 212,8. 18 ± 2. 136,8.20 ± 4. 266, the expression of COX-2 was no related to pathological grade( P 〉 0.05). Condusions The expressions of TLR4, COX-2 and EP2 were significantly upregulated in the intestinal epithelium of active UC; also there was a direct positive correlation among the expressions of TLR4, COX-2 and EP2
