摘要
目的探讨宫颈液基细胞学阴性的人乳头状瘤病毒(HPV)感染病毒负荷量、年龄因素与宫颈病变的关系。方法 2011年5月—2012年12月,我院黄岛院区采用液基薄层细胞学(LCT)联合第二代杂交捕获(HC2)技术检测HPV-DNA进行宫颈癌机会性筛查,选取LCT阴性、HPV阳性病人575例,均行阴道镜下宫颈活检且有病理诊断。分析年龄、HPV病毒负荷量与病变级别之间的关系。结果 575例病人中宫颈活检病理诊断宫颈浸润癌1例,宫颈上皮内瘤变(CIN)3、CIN2、CIN1分别为26、17、46例,慢性宫颈炎485例。30~49岁组≥CIN3的患病风险是≥50岁组病人的1.556(95%CI0.357~6.776)倍;HPV病毒高负荷量组≥CIN3的患病风险是低负荷量组的2.017(95%CI0.760~5.350)倍。30~49岁组HPV病毒低、中、高负荷量者≥CIN2的患病率分别为3.2%、8.4%和10.5%,差异具有统计学意义(χ2=6.684,P<0.05)。结论在LCT阴性、HPV阳性病人中,30~49岁年龄组宫颈高度病变患病率明显增加,且病毒负荷量越高,患病率越高。
Objective To study the association of HPV load, patient age with lesions of cervix in patients who had a neg- ative liquid-based cytology and positive HPV. Methods During 05.2011--12.2012, a combined LCT and HC2 technique was em- ployed (in our Huangdao zone branch hospital) to detect HPV-DNA for opportunistic screening of cervical cancer,575 patients with LCT-negative and HPV-positive were selected. A biopsy under colposcope was done. The relationship between age, HPV load and grade of the disease was analyzed. Results Of the 575 patients underwent biopsy, invasive cervical cancer was confirmed in one case, cervical intraepithelial neoplasia (CIN) 3, CIN2 and CIN1 were 26,17 and 46, respectively, and 485 with chronic cervicitis. The risk to be suffered from CIN3 in 30--49 year-old group was 1.556 fold of those in ≥50-year-old group (95MCI 0.257-- 6.776), and that in high-HPV load group was 2.017 times higher than low HPV-load group (95%CI 0.760--5.350). In the 30-- 49-year-old group, the prevalence of≥CIN2 in low-, moderate- and high-HPV- was 3.2%,8.4%, and 10.5%, respectively, the differences being significant (x2=6.684,P〈0.05). Conclusion In patients with LCT negative and HPV positive, the preva- lence of CIN in 30--49 year old group is obviously increased, the higher the viral load is, the higher the prevalence will be.
