摘要
目的探讨右美托咪定对上腹部手术患者七氟烷呼气末有效浓度的影响。方法选取我院于2010年5月~2012年10月收治的80例接受上腹部手术的患者,随机分为对照组与实验组,每组40例。对照组在麻醉诱导时,单纯吸入七氟烷进行维持;实验组吸入与对照组相同剂量的七氟烷维持麻醉,并于麻醉诱导前后分别静脉滴注右美托咪定。观察两组患者的血流动力学及七氟烷呼气末的有效浓度差异。结果手术开始前、气管插管前实验组与对照组患者的平均动脉压与平均心率的比较差异无统计学意义(P>0.05);气管插管后10 min,开腹时、关腹时及手术结束时,实验组患者的平均动脉压与平均心率要明显低于对照组患者,差异有统计学意义(P<0.05);气管插管后10 min,切开皮肤时,切开皮肤20 min后,切开皮肤40 min后,关腹时及手术结束时,实验组患者的七氟烷呼气末有效浓度要明显低于对照组,差异有统计学意义(P<0.05)。结论右美托咪定能够有效降低上腹部手术患者七氟烷呼气末有效浓度,且能够使患者的血流动力学处在较为稳定的状态,值得在全麻手术中推广。
Objective To study effect of dexmedetomidine on end-tidal sevoflurance concentration in patients undergoing elective upper abdominal operation.Methods From May,2010-October,2012,80 patients undergoing elective upper abdominal in our hospital were randomly divided into the control group and the experimental group.There were 40 cases in each group.The anesthesia of control group was maintained with sevoflurance inhalation alone while that of the experimental group took in the same doze as the control and administer the dexmedetomidine by intravenous infusion before and after anesthesia induction.Observe the difference of the hemodynamics and end-tidal sevoflurance concentration between the two groups.Results The difference of MAP and HR when the operation began and before intubation between the control group and the experimental group was not statistically significant(P&gt;0.05);10 minutes after the intubation,when the abdominal was explored and closed,at the end of the operation,the MAP and HR between of the experimental group was obviously lower than those of the control group(P&lt;0.05).10 minutes after the intubation,skin incision,20 minutes and 40 minutes after the skin incision,the end-tidal sevoflurance concentration of the experimental group was obviously lower than that of the control group(P&lt;0.05).Conclusion Dexmedetomidine can effective lower the end-tidal sevoflurance concentration in patients undergoing elective upper abdominal and make the hemodynamics of the patients in a relatively stable state,so it is worth promoting.
