心电图在判定急性下壁心肌梗死罪犯血管的意义

摘要 目的 评定心电图在判定急性下壁心肌梗死罪犯血管的意义。方法 对56例急性下壁心肌梗死患者的心电图及冠状动脉造影资料进行分析。结果①罪犯血管是右冠状动脉占85.7%,回旋支占14.3%;②窦性心动过缓、房室传导阻滞、室颤是右冠状动脉闭塞指标;③STI.aVL压低≥1.0mm是右冠状动脉闭塞敏感指标(P<0.05及P<0.01),敏感性及特异性分别为69%、75%及92%、75%,而缺乏STI.aVL压低是回旋支闭塞的敏感指标(P均<0.01),敏感性、特异性各为75%、69%及75%、92%;④STV5V6压低≥1.0mm预示多支病变(P<0.01),敏感性、特异性各为75%、81%,且提示合并前降支病变(P<0.01),其敏感性、特异性各为82%、82%。结论 急性下壁心肌梗死时心电图对判定罪犯血管有重要预测价值。 Objective To investigate the value of the electrocardiogram in predicting the culpril artery in inferior wall acute myocardial infarction (IAMI). Method Fifty-six patients with LAMI were studied. Results(1) IAMI was caused by right coronary artery (RCA) in 48 (85.7%) cases, by left circumflex artery (LCX) in 8 (14.3%) cases. (2)Sinus bradycardia. Atroventricular block, ventricular fibrillation identified RCA occlusion. (3)Significants ST segment depressions ( ST≥ 1 mm) in leads I and aVL were more common m RCA obstruction (P<0.05 and P<0.01) with a sensitivity of 69%,92%, a specificity of 75%, 75%, whereas the lack of ST segment depression in these leads indicates LCX, with a sensitivity of 69%, 92%(P<0.01).(4)ST segment depression( ST≥ 1 mm) in leads V5 and V6 are predictive of multivessel disease (P <0.01) with a sensitivity) of 75% and a specificity of 81%, predictive of involving the left anterior descending artery (LAD) in particular (P <0.01) with a sensitivity of 64% and a specificity of 82%. Conclusion Electrocardiogram has significant value in predicting the culpril artery and the location of occlusion in inferior wall acute myocardial infarction.
