宫内窘迫新生儿心肌酶活性的临床研究

摘要 目的:通过分别检测宫内窘迫有窒息和无窒息患儿治疗前后心肌酶水平的变化,研究宫内窘迫患儿心肌缺氧性损伤状况及各项心肌酶活性指标测定的临床意义。方法:于生后第1天及第3天,将宫内窘迫患儿分为生后有窒息组和无窒息组,用全自动生化分析仪测定其血清天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、α-羟丁酸脱氢酶(α-HBDH)活性,与同期出生的无宫内窘迫新生儿进行比较分析。结果:与对照组相比,宫内窘迫无窒息组AST、CKMB有显著性增高,LDH、CK、α-HBDH无显著性变化;窒息组所有心肌酶指标活性均呈显著性增高,缺氧程度越重,心肌酶活性增高越明显;治疗后第3天,除窒息组LDH外,其余指标基本恢复到正常水平。结论:宫内窘迫患儿存在缺氧性心肌损伤,经产前、产时及时处理,出生无窒息者心肌损害明显小于窒息患儿。血清心肌酶水平能及时反映宫内窘迫新生儿心肌损伤的程度,尤以CK-MB、AST较灵敏,对判断病情、指导治疗有重要的临床意义,宫内窘迫患儿经及时治疗预后良好。 Objective To explore the changes and clinical significance of serum myocardial enzyme in the newborns with fetal distress.Method Detected the activities of serum myocardial enzyme by Beckman Biochemistry Analyzer at the first day and the third day.The neonates with fetal distress were divided into two groups,the asphyxia group and without asphyxia group.The myocardial enzyme include:serum aminotransferase(AST),lactate dehydrogenase(LDH),creatine kinase(CK),creatine kinase isoenzyme(CK-MB),α-hydroxybutyrate dehydrogenase(α-HBDH),ect.Results Part of index of myocardial enzyme(AST、CK-MB) have the statisticaly changes in the fetal distress neonates without asphyxia,in the asphyxia group,all of the serum myocardial enzyme levels were significantly higer than those of the control group.Through therapy,the activity of serum myocardial enzyme recover to the normal level in three days.Conclusion The detection of AST、LDH、CK、CK-MB、α-HBDH is helpful to evaluate the degree of anoxic cardial cell damage.The myocardial enzyme can be regarded as useful indicatiors to judge the severity in the newborns with fetal distress.Pay attention to the perinatal health is very important to improve the prognosis.
