摘要
目的分析职业噪声性聋的畸变产物耳声发射(distortion product otoacoustic emissions,DPOAE)及纯音听阈的特点,探讨DPOAE检查在职业噪声性聋早期检测中的作用。方法对15例正常人(30耳,对照组)和36例噪声暴露的某电厂工人(69耳,实验组)进行DPOAE及纯音测听检查;其中实验组依据纯音测听结果分为听阈正常组及听力下降组。比较分析各实验组和对照组的纯音听阈和DPOAE幅值。结果①听力下降实验组的纯音听阈在2~8 kHz明显提高,与对照组均有显著的统计学差异(P<0.05或P<0.001);②实验组及听力下降实验组的DPOAE幅值在1.5~8 kHz较对照组均明显下降并有显著的统计学差异(P<0.05或P<0.001);③听力正常实验组的DPOAE幅值在4~8 kHz较对照组有统计学差异(P<0.05)。结论 DPOAE在职业噪声性聋的早期诊断和检测方面可能有更重要作用,4~8 kHz可能为噪声性聋早期易受损频率区。
OBJECTIVE To analyse the pure tone threshold (PTA) and distortion product otoacoustic emissions (DPOAE) features in occupational noise- induced hearing loss, and investigate the role of DPOAE in the early identification of occupational noise-induced hearing loss (NIHL) . METHODS PTA and DPOAE were performed both in 15 (30 ears, control group) health young adults and 36 (69 ears, experimental group) workers from the power plant who have been working in loud sounds environment. According to the result of PTA, the experimental group was divided into the normal hearing group and the hearing loss group. The PTA thresholds and amplitudes of DPOAE were compared and analyzed. RESULTS 1. The PTA of hearing loss group at 2-8 kHz are extremely increased (P 〈 0.05 or P 〈 0.001) ; 2. Both the DPOAE amplitudes of the total experimental group and the hearing loss group at 1.5-8 kHz were lower than that of the normal controls (P〈0.05 orP〈0.001) ;3. The DPOAE amplitudes of normal hearing group at 4-8 KHz were drcreased too (P〈0.05) . CONCLUSION The DPOAE has an important role in early diagnosis and detection of occupation NIHL, and the 4-8 kHz may be the earliest frenquency area to be damaged.
