摘要
目的探讨血清脂联素水平与湖南汉族人群高尿酸血症的关系。方法采用整群随机抽样法,抽取2008年10月至12月在我院体检的249名湖南汉族人为研究对象,进行问卷调查、体格检查及生化指标测量等检查。所有数据采用t检验,Pearson相关分析、多元线性回归分析等进行统计学处理。结果湖南汉族高尿酸血症组与血尿酸正常组的年龄、腰围、甘油三酯、收缩压、舒张压、血尿酸水平、体质量指数(BMI)及脂联素差异均有统计学意义,其中高尿酸血症组的血清脂联素水平(5.0±2.7)μg/ml显著低于血尿酸正常组(6.8±4.2)μg/ml,差异有统计学意义(t=3.961,P〈0.05);且在血尿酸正常组中,女性脂联素水平显著高于男性(t=4.99,P〈0.05)。相关性分析显示,脂联素与血尿酸呈负相关,与年龄、血压无相关性。用逐步回归筛选变量,结果表明血尿酸、性别、BMI是影响脂联素最显著的因素(P〈0.05)。结论血清脂联素水平下降可能为高尿酸血症的发病机制之一,检测脂联素水平可为高尿酸血症的防治提供依据。
Objective To explore the relationship between serum adiponectin concentration and hyper- uricemia in Han people in Hunan. Methods Cluster random sampling method was used to carry out the survey during October to December 2008 in Changsha Health Checkup Center of the Second Xiangya Hospital. All subjects completed the questionnaires, physical examination, biochemical measurements, and the data were analyzed by t test, Pearson's correlation analysis and multiple linear regression analysis. Results The Age, WC, TG, SBP, DBP, SUA, BMI were significantly higher in hyperuricemia group compared with the normal uric acid group. The concentration of adiponectin in hyperuricemia group was significantly lower than that in the normal uric acid group (5.0±2.7 vs 6.8±4.2 μg/ml, t=3.961, P〈0.05). In the normal serum uric acid group, the serum adiponectin concentration in female was significantly higher than that in male (t=4.99, P〈0.05 ). Pearson's correlation analysis showed that serum adiponectin concentration was negatively correlated with the serum uric acid level, but the adiponectin level was not significantly correlated with Age and blood pressure. Stepwise analysis showed that the main factors that could affect the adiponectin level were SUA, gender and BMI (P〈0.05). Conclusion The decreasing of serum adiponectin concentration might be one of the mechanisms of hyperuricemia, therefore, detecting serum adiponectin concentration may provide the basis for the prevention and treatment of hyperuricemia.
