摘要
目的比较改良腹腔镜下腹腔内网片植入术(IPOM)与腹腔镜下IPOM治疗老年腹股沟疝手术方法,探讨更加合理的手术方式。方法回顾分析该研究62例老年腹股沟疝患者,其中34例观察组实施改良腹腔镜下IPOM,另28例对照组实施腹腔镜下IPOM,对比分析两组术后恢复活动时间、术后并发症(包括术后持续性疼痛、阴囊气血肿、尿潴留)以及术后复发率等情况。结果观察组与对照组手术后恢复活动时间、术后并发症比较,差异无显著性(P>0.05);两组病例均随访2年,观察组无复发病例,对照组复发5例。结论改良腹腔镜下IPOM修补腹股沟疝手术效果好、复发率低,是目前治疗老年腹股沟疝较理想的术式之一,老年高危腹股沟疝患者应首选改良腹腔镜下IPOM术式。
[Objective] To compare laparoscopic modified IPOM and IPOM under the treatment of elderly patients with inguinal hernia operation method, operation of a more reasonable way. [Methods] 62 cases of elderly patients with inguinal hernia in our hospital were reviewed, including 34 cases of observation group received the modified laparoscopic IPOM, another 28 cases in control group of laparoscopic IPOM, comparative analysis was made of the recovery period, the complications after operation in two groups （including the persistent pain, hematoma of scrotum gas, postoperative urinary retention） and operation the recurrence rate. [Results] The observation group compared with the control group, operation effect, no significant difference （P 0.05）; Two cases were followed up for 2 years, no recurrent cases in observation group, the control group of 5 cases of recurrence. [Conclusion] The modified laparoscopic IPOM inguinal hernia repair operation and good effect, low recurrence rate, is the ideal surgical treatment of inguinal hernia in the elderly, the elderly patients with inguinal hernia should be the first choice of modified laparoscopic IPOM surgery.
