摘要
目的比较阿立哌唑与其他非典型抗精神病药治疗社区稳定期精神分裂症患者的长期疗效。方法入组95例服用非经典抗精神病药物的患者,阿立哌唑治疗组44人,利培酮、喹硫平和奥氮平单药治疗共51例。采用阳性和阴性症状量表(PANSS)、自知力与治疗态度问卷(ITAQ)和药物依从性量表在入组时、训练期末各测评1次;随访期内每12周测评1次,随访总时间为52周。结果自第28周末起,研究组患者的PANSS总分、阴性量表分和一般精神病理量表分与对照组比较,差异有显著性(P<0.05)。随访中,两组患者PANSS阳性量表分差异无显著性(P>0.05)。第28周末起研究组的ITAQ分值明显高于对照组,研究组患者对药物的依从程度要好于对照组,经统计学检验差异有显著性(P<0.05)。研究组3例复发(9.1%),对照组6例复发(11.7%),2例再住院(3.9%)。研究组的复发率和再住院率均低于对照组,但两组之间差异无显著性(Fisher精确概率χ2=0.178,P>0.05)。结论阿立哌唑与利培酮、喹硫平和奥氮平比较,在控制患者阳性症状上有相似疗效,但是在改善患者阴性症状和安全性、耐受性以及药物依从性上有很大优势。
[ Objective ] To compare the long-term effect of Aripiprazole and other atypical antipsychotic drugs in the schizophrenia in the community. [ Method ] 95 patients with atypical antipsychotic drugs were included. Patients were divided into 2 groups. The intervention group was administered aripiprazole, another control group was adminis- tered risperitone, olanzapine and quetiapine monotherapy. The Scales included PANSS （Positive and Negative Syn- drome Scale）, ITAQ （Insight and Treatment Attitudes Questionnaire） and Drug Compliance scale. The assessment was made at the basement, every 12 weeks and at the end of 52 weeks follow up visit. [Results] PANSS total score, Negative Syndrome score and General psychopathology scale score in the aripiprazole group were superior to that in control group significantly （P 〈0.05） from the 28th week. Positive syndrome scale scores were no significant different in 52 follow up visits （P 〉0.05）. Also, ITAQ and Drug Compliance scale scores were obtained better in the interven- tion group （P 〈0.05）. 6 patients relapsed and 2 patients return to hospital in the control group, Meanwhile 3 patients relapsed in the intervention group. But 2 groups were no significant different （Fisher Exact probability X2=0.178, P 〉 0.05）. [ Conclusion] Aripiprazole has the same efficacy with risperitone, olanzapine and quetiapine. But Aripiprazole is superior in the cure of negative syndrome and more safety and toleration.
