摘要
目的检测分析鲍曼不动杆菌的耐药性、全面了解多重耐药鲍曼不动杆菌β-内酰胺类耐药机制;方法药敏试验为Kirby-Bauer法、基因检测采用PCR对20株多药耐药鲍曼不动杆菌进行了22种β-内酰胺酶基因和膜孔蛋白carO基因检测;结果20株鲍曼不动杆菌耐药率除亚胺培南外,其余药物耐药率均在95%以上、共有TEM、PER、ADC等3种β-内酰胺酶基因呈阳性,阳性率分别为95.0.0%(19/20)、25.0%(5/20)、100.0%(20/20)。膜孔蛋白carO基因突变率达100.0%;结论本组MDR-ABA菌多种β-内酰胺类药物耐药与产TEM、PER、ADC等3种β-内酰胺酶相关外,还与膜孔蛋白编码基因carO突变有关。
Objective To investigate the resistance spectrum and the resistance mechanisms of β-1actams antibiotics in Acinetobacter baumannii. Methods Susceptibility test was performed by K-B method, while 22 β-1actamase genes and the gene of a outer membrane protein(OMP) carO were detected by polymerase chain reaction(PCR) with 20 stains of multiple drug resistant Acinetobacter baumannii (MDR-ABA). Results Except for imipenem, the drug resistance rate of the 20 stains Acinetobacter baumannii were more than 95%. Three kinds of β-1actamase genes (TEM, PER, ADC)were positive among the 20 stains Acinetobacter baumannii, and the positive rate were 95.0%(19/20), 25.0%(5/20) and 100.0%(20/20) respectively. The mutation rate of the carO gene was 100.0%. Conclusions The expression of the three kinds of β-1actamase genes (TEM, PER, ADC) and the mutation of the outer membrane protein carO gene were the impotant mechanisms for the resistance of β-1actams antibiotics in the 20 stains of MDR-ABA.
