摘要
目的探讨C-反应蛋白(CRP)检测在小儿细菌性肺炎和支原体肺炎鉴别诊断中的价值,为小儿肺炎的鉴别诊断提供参考。方法随机选取2010年9月-2012年9月医院儿科收治的细菌性肺炎患儿(细菌性肺炎组)和支原体肺炎患儿(支原体肺炎组)、门诊进行体检健康儿童(对照组)各80例,检测CRP变化。结果 CRP水平:对照组为(3.24±0.45)mg/L,细菌性肺炎组治疗前为(44.03±15.83)mg/L、治疗3d后为(15.12±6.21)mg/L,支原体肺炎组治疗前为(13.97±4.96)mg/L、治疗3d后为(5.29±2.33)mg/L,治疗前细菌性肺炎组、支原体肺炎组明显高于对照组(P<0.05),细菌性肺炎组高于支原体肺炎(P<0.05),治疗3d后细菌性肺炎组高于支原体肺炎(P<0.05),细菌性肺炎组、支原体肺炎组组内治疗前后比较差异存在统计学意义(P<0.05);CRP细菌性肺炎组治疗前阳性率为100.00%、治疗3d后阳性率为52.50%,支原体肺炎组分别为66.25%、21.25%,细菌性肺炎组、支原体肺炎组组内治疗前后阳性率比较差异存在统计学意义(P<0.05),组间治疗前后阳性率比较差异存在统计学意义(P<0.05)。结论 CRP在小儿细菌性肺炎中阳性率最高,检测CRP有助于鉴别细菌性肺炎与支原体肺炎。
OBJECTIVE To study the clinical value of C-reactie protein(CRP)detection in diagnosis of the pediatric bacterial pneumonia and the Mycoplasma pneumonia so as to guide the diagnosis of infantile pneumonia.METHODS The children with bacterial pneumonia(the bacterial pneumonia group)and the children with Mycoplasmapneumonia(the Mycoplasmapneumonia group),who were treated in the pediatrics department from Sep 2010to Sep 2012,were enrolled in the study,the healthy children receiving the medical examination(the control group)were also selected,with 80cases in each group,then the change of CRP level was determined.RESULTS The CRP level of the control group was(3.24±0.45)mg/L,the bacterial pneumonia group before treatment(44.03±5.83)mg/L,the bacterial pneumonia group 3days after the treatment(15.12±6.21)mg/L,the Mycoplasma pneumonia group before treatment(13.97±4.96)mg/L,the Mycoplasma pneumonia group 3 days after the treatment(5.29±2.33)mg/L,the CRP level of the bacterial pneumonia group and the Mycoplasma pneumonia group before the treatment was significantly higher than that of the control group(P〈0.05),the CRP level of the bacterial pneumonia group was higher than that of the Mycoplasmapneumonia group(P〈0.05),the CRP level of the bacterial pneumonia group 3days after treatment was higher than that of the Mycoplasma pneumonia group(P〈0.05),the difference in the CRP level before and after the treatment between the bacterial pneumonia group and the Mycoplasma pneumonia group was statistically significant(P〈0.05).The positive rates of the CRP of the bacterial pneumonia group were 100.00% before the treatment and 52.50% 3days after the treatment,which were respectively 66.25%and 21.25%in the Mycoplasmapneumonia group,the difference in the positive rate before and after the treatment between the bacterial pneumonia group and the Mycoplasma pneumonia group was statistically significant(P〈0.05).CONCLUSIONThe positive rate of CRP is the highest in children
