摘要
目的探讨鼓室内灌注地塞米松治疗主观性耳鸣的效果。方法选择病史在1个月以上已接受过药物治疗或未治疗的主观性耳鸣患者120例(139耳),地塞米松组(A组,49耳)鼓室灌注地塞米松磷酸钠注射液,同时口服扩血管药及及营养神经药;生理盐水组(B组,45耳)外耳道骨壁注射生理盐水,同时口服扩血管药及营养神经药;对照组(C组,45耳)仅口服扩血管药及营养神经药。所有患者在治疗前、治疗结束时均分别行纯音测听和耳鸣问卷调查,比较疗效,随访6个月。结果 120例(139耳)均完成全部的治疗及随访。治疗结束时,A组31耳有效,18耳无效,总有效率63.30%(31/49);B组19耳有效,26耳无效,总有效率42.20%(19/45);C组17耳有效,28耳无效,总有效率37.80%(17/45);6个月后3组的耳鸣控制率分别是61.90%、41.90%和36.80%。结论鼓室内灌注糖皮质激素治疗主观性耳鸣患者是有效的,可作为常规药物治疗无效患者的另一种治疗选择。
【Objective】 To study the efficacy of dexamethasone injection for the subjective tinnitus.【Method】 A prospective study was designed to compare the efficacy of intratympanic dexamethasone injection for subjective tinnitus.120 cases（139 ears） with subjective tinnitus for more than one month and treated by conservative therapies or untreated were involved.The patients were randomized into 3 groups： 49 ears were included in group A,the dexamethasone group,with intratympanic dexamethasone sodium phosphate injection and oral intake of vasodilator and neurotrophic drugs at the same time;45 ears were included in group B,the saline group,with external auditory meatus bone normal saline injection,and meanwhile an oral intake of vasodilator and neurotrophic drugs;and group C,the control group included 45 ears with oral intake of vasodilator and neurotrophic drugs only.Pure tone audiogram and tinnitus matching were tested before and after the course of treatment immediately,and were tested again after half a year’s following up.【Results】 All 120 cases,（139 ears） accepted the whole treatment and were followed up for half a year successfully.The effective rate of the dexamethasone group,the saline group and the control group was 63.30%（31/49）,42.20%（19/45）,37.80%（17/45） respectively;the control rate of half a year after the treatment was 61.90%,41.90%,36.80% respectively.【Conclusion】 Intratympanic glucocorticoid injection has a positive effect on subjective tinnitus and may be considered to be an alternative treatment to subjective tinnitus.
