摘要 目的采用Meta分析方法评价尤瑞克林治疗急性脑梗死的有效性和安全性。方法计算机检索TheCochrane Library、PubMed、SCI、EMbase、CBM、CNKI、VIP和WanFang等数据库,收集有关尤瑞克林治疗急性脑梗的随机和半随机对照试验,采用RevMan 4.2软件进行Meta分析。结果共纳入29个随机对照试验研究,2926例患者,其中治疗组1591例,对照组1335例。Meta分析结果显示尤瑞克林组疗效较对照组显著提高,有效率[OR=3.06,95%CI(2.45,3.81),P<0.01];显效率[OR=1.52,95%CI(1.42,1.68),P<0.01],差异有统计学意义。尤瑞克林不良反应主要为面部潮红,球结膜充血及血压轻度下降,多可自行恢复。结论现有的证据表明尤瑞克林能有效地改善急性脑梗死的神经功能缺损,且无明显严重不良反应。受纳入研究质量所限,上述结论仍需更多大样本、多中心、前瞻性的随机对照研究加以验证。 Objective To evaluate the effectiveness and security of Urinary Kallidinogenase for acute cerebral infarction through Meta-analysis. Methods Databases including the Cochrane Library, PubMed, SCI, EMbase, CBM, CNKI, VIP and WanFang Data were searched. Relevant randomized controlled trials and Quasi-randomized controlled trials on patients with acute cerebral infarction and treated with Urinary Kallidinogenase were collected to perform Meta analysis using RevMan 4.2. Results Twenty nine studies involving a total of 2926 cases (trial group: 1591 cases; control group: 1335 cases) were included. Results of Meta-analysis showed the outcome of patients in trial group was much better than those in control group: The total effective rate [OR=3.06, 95%CI (2.45 to 3.81), P 〈 0.01] and obvious effective rate [0R=1.52, 95%CI (1.42 to 1.68), P 〈 0.01] were significantly improved. Significant differences between the two groups were observed. Main adverse reactions of Urinary Kallidinogenase were facial flushing, conjunzrival hyperemia and blood pressure decreased slightly, most of which can recover on its own. Conclusion Urinary Kallidinogenase showed excellent efficacy in improving defected neurological function for patients with acute cerebral infarction, and few serious adverse reaction. However, due to the limitation of the quality of the included studies, the above conclusion should be tested by conducting more large-scale, multi-center and prospective RCTs in future.
