剖宫产孕妇再次妊娠子宫下段肌层厚度研究 被引量:21

摘要 目的应用高频超声观测有剖宫产史的晚期妊娠妇女子宫下段肌层厚度,探讨其对分娩方式选择的指导意义。方法晚期妊娠妇女353例,按既往有无剖宫产史分为剖宫产组(181例)和对照组(172例,无剖宫产史)。均于孕31~33周和孕37~39周进行超声检查,应用高频线阵探头测量子宫下段肌层厚度。结果孕37~39周时两组子宫下段肌层厚度均低于孕31~33周时的厚度(P均<0.01);剖宫产组孕31~33周及孕37~39周时子宫下段肌层厚度均低于对照组(P均<0.01)。以1.6 mm为界,孕37~39周时,剖宫产组中有68例(39.53%)子宫下段肌层变薄(≤1.6 mm);对照组17例(9.39%),两组比较差异有统计学意义(P<0.01)。结论既往有剖宫产史的孕妇妊娠晚期子宫下段肌层明显变薄。应用高频线阵探头进行超声检查测量子宫下段肌层厚度有助于临床对分娩方式作出选择。
