摘要
目的回顾性分析放射治疗眼眶内炎性假瘤的效果。方法自2007年4月～2011年1月对38例激素治疗后复发／或进展的炎性假瘤患者实施单前野铅挡块遮挡晶体的x线外照射放射治疗，剂量计算点为射野中心轴上根据具体患者MRI影像测定的最大眼眶深度，中位处方剂量26Gy（20—30Gy）。结果38例患者29例为激素治疗后复发，9例为激素治疗后无效或病情进展。38例患者放射治疗后无效1例，症状和体征改善6例，症状和体征完全消失31例。中位随访时间23个月（10～45个月），完全反应者未发现初始症状复发，部分反应者病情稳定无进展，无效1例病情无进展。急性期反应不明显，后期放射反应有轻度眼睛干燥症5例，无放射诱发的白内障发生。结论使用单前野铅挡块遮挡晶体X线外照射治疗眼眶内炎性假瘤效果肯定，急性期和后期放射副作用不明显。
Objective To review institutional outcomes for patients treated with external-beam radiotherapy（RT） for or- bital pseudotumor. Methods Thirty-eight patients with orbital pseudotumor were treated with a lens blocked single an- terior field between April 2007 and Jan 2011 median prescribed dose 26Gy （20-30Gy）.Response to RT was evaluated at 1 month post RT. Results In thirty-eight orbital pseudotumor patients, there were twenty-nine patients who were initially treated with cortic6steroids and had recurrence after steroids cessation; there were nine patients had no re- sponse to steroid. Thirty-one patients achieved a complete response （CR） and ceased corticosteroids therapy after RT. Six patients obtained a partial response （PR） and reduced corticosteroid dose. One patient had no response. Median fol- low-up was 23 months （10-45 months）, no recurrence observed in CR patients, the other patients preserved no progres- sion. Late effect of chronic dry eye was seen in five patients. Cataract induced by RT had not been observed. Conclu- sion Radiotherapy with a lens blocked single anterior field is an effective treatment for acute symptomatic improve- ment and long-term control of orbital pseudotumor. Late effects are acceptable.
