全腹部64层螺旋CT(MSCT)增强扫描诊断创伤所致腹部脏器损伤的临床应用 被引量:17

摘要 目的观察全腹部64层螺旋CT(MSCT)增强扫描诊断创伤所致腹部脏器损伤的临床应用价值。方法选取丽水市中心医院治疗的腹部创伤患者957例,利用64层螺旋CT增强扫描,并将CT的诊断结果与手术、治疗结果进行对照,观察MSCT增强扫描诊断创伤所致腹部脏器损伤的特异性和敏感性。结果 CT的诊断结果与术后、治疗的结果相比,最后发现在实质脏器的损伤诊断中假阳性为5例、假阴性为3例;而在空腔脏器的CT诊断结果上假阳性为4例、假阴性为3例。CT诊断实质脏器损伤的灵敏度为97.86%,特异性为99.59%;CT诊断空腔脏器损伤的灵敏度为76.67%,特异性为99.89%。结论全腹部MSCT增强扫描诊断腹部实质脏器损伤的特异性与诊断腹部空腔脏器损伤的特异性无显著统计学差异(P>0.05),但是其诊断腹部实质脏器损伤的灵敏度却要显著高于空腔脏器损伤的灵敏度(P<0.05)。 Objective To observe the application value of enhancement scan of 64 layer spiral CT (MSCT) on whole abdomen in the diagnosis of trauma caused by abdominal visceral injury. Methods Selected 957 cases of abdominal trauma patients admited in the Central Hospital of Lishui City,used the enhancement scan of 64 layer spiral CT and compared the CT diagnosis results with surgery and treatment results,so as to observe the specificity and sensitivity of MSCT for diagnosis of trauma caused by abdominal visceral injury. Results Compared with surgery and treatment re- suits,the CT diagnosis finally found false positives for 5 cases, false negative for 3 cases in damage detection of solid organ; false positives for 4 cases, false negatives for 3 cases in the hollow organ. The sensitivity of CT in the diagnosis of solid organ injuries was 97.86%, specificity was 99.59%,and the sensitivity of CT in the diagnosis of hollow organ injuries was 76.67%, specificity was 99.89%. Conclusion Compared the specificity of enhancement scan of MSCT on whole abdomen of solid organ injuries and hollow organ injuries, there was no statistically significant difference (P 〉 0.05),but the sensitivity of solid organ injuries was significantly higher than the hollow organ injuries(P 〈 0.05).
