2型糖尿病动脉血管病变与血小板的关系研究 被引量:5

摘要 目的:观察2型糖尿病(T2DM)不同动脉血管病变中血小板计数、血小板平均体积(MPV)及血小板分布宽度(PDW)的变化情况。方法:将2012年1月至2012年10月沈阳医学院奉天医院收治的180例T2DM患者作为研究对象。根据踝臂指数(ABI)和颈动脉内膜中层厚度(IMT)分为单纯ABI≤0.9组(A组)45例、单纯IMT>0.8 mm组(B组)46例、ABI≤0.9且IMT>0.8 mm组(C组)39例、正常组(D组,ABI>0.9且IMT≤0.8 mm)50例,比较4组患者血小板计数、MPV及PDW变化。结果:A、B、C组MPV分别为11.96±0.57,11.22±1.42,12.26±0.86,与D组(9.67±1.56)比较,差异有统计学意义(P<0.05),而A、B、C组间比较差异无统计学意义(P>0.05);4组PDW和血小板计数比较,差异无统计学意义(P>0.05)。结论:T2DM动脉血管病变存在MPV增加,应加强MPV动态观察。 Objective:To investigate the alterations in platelet counts, mean platelet volume (MPV) and platelet distribution width (PDW) in type 2 diabetes mellitus (T2DM) complicated with arterial lesions. nethods:A total of 180 T2DM patients registered to Fengtian Hospital of Shenyang Medical College between January 2012 and October 2012 were included in this study. Based on the ankle brachial index (ABI) and intima-media thickness (IMT) of the carotid artery, these patients were assigned to 4 groups: group A (ABI ≤0. 9 andlMT≤0.8 mm, n=45), group B (ABI〉0.9 and IMT〉0.8 mm, n=46), group C (ABI≤0.9 and IMT 〉0.8mm, n = 39) and a normal group D (ABI 〉0.9 and IMT≤0.8 mm, n = 50). The values of platelet counts, MPV and PDW were measured and compared across these groups. Results :The values of MPV in groups A ( 11.96 ±+ 0.57), B ( 11.22 ± 1.42) and C ( 12.26 ± 0.86) did not differ significantly among one another ( all P 〉 0. 05 ), but were lower than the value in the normal group D (9.67 ± 1.56) ( P 〈 0. 05 ). There were no statistical differences in platelet counts and PDW among the four groups ( all P 〉 0.05 ). Conclusion : The elevated value of MPV as detected in T2DM complicated with arterial lesions may justify a serial monitoring of this parameter in the patients.
