综合康复治疗联合叶酸、甲钴胺对脑梗死患者血同型半胱氨酸水平的影响 被引量:11

Effect of Comprehensive Rehabilitation with Folic Acid and Mecobalamin on Blood Homocysteine Levels in Cerebral Infarction

摘要 目的探讨综合康复治疗联合叶酸及甲钴胺注射液对脑梗死患者血同型半胱氨酸(Hcy)和功能障碍的影响。方法将88例伴高同型半胱氨酸血症的脑梗死患者随机分为对照组和康复治疗组,对照组给予常规口服阿司匹林片、叶酸片,肌肉注射甲钴胺注射液,康复治疗组在应用药物基础上加行脑梗死后综合康复治疗项目,包括针刺、运动疗法、作业疗法、理疗、认知功能训练等。入院24h内和治疗2周后测定患者血Hcy水平及改良Barthel指数(MBI)并行相关分析。结果两组血Hcy水平均较治疗前明显下降,MBI评分均较治疗前升高,但康复治疗组较对照组改善明显(P<0.05)。血清Hcy水平和MBI评分呈负相关性(P<0.05)。结论综合康复治疗联合叶酸、甲钴胺注射液治疗可有效降低脑梗死患者血Hcy水平,提示综合康复治疗不仅能够促进患者功能障碍的恢复,同时可以减轻高同型半胱氨酸血症对脑血管的毒性作用,有助于脑梗死患者的恢复和二级预防。 Objective To investigate the effects of comprehensive rehabilitation treatment with folic acid and mecobalamine injec- tion on blood homocysteine and dysfunction condition in patients with acute cerebral infarction. Methods Eighty - eight cases of cere- bral infarction with hyperhomocysteinemia were randomly divided into control group and rehabilitation group. All patients were given oral aspirin tablets, folic acid tablets, and intramuscular injection of mecobalalnin injection. Patients in rehabilitation treatment group were given comprehensive rehabilitation treatment including acupuncture, occupational therapy, physical therapy, and cognitive func- tion training. Within 24 h after admission and 2 weeks after treatment, serum homocysteine levels and modified Barthel index (MBI) were detected. Results Blood homocysteine levels in two groups were decreased significantly,especially that in treatment group was de- creased significantly (P〈0.05). MBI scores in two groups were high, especially that in the treatment group significantly (P〈0.05). The level of serum homocysteine and MBI scores had negative correlation (P〈0.05). Conclusion Comprehensive rehabilitation treat ment combined with folic acid, and mecobalamin injection could effectively reduce blood homocysteine levels in patients with cerebral infarction than that using folic acid, mecobalamin alone. The result suggested that comprehensive rehabilitation treatment could not only promote the recovery of patients with dysfunction,also could alleviate the toxic effects of hyperhomocysteinemia on cerebral ar tery,contribute to the recovery of patients with cerebral infarction.
