摘要
目的:观察慢性坐骨神经压迫损伤(chronic constriction injury,CCI)大鼠接受脉冲射频(pulsedradiofrequency,PRF)治疗后疼痛行为学变化和背根神经节(dorsal root ganglion,DRG)中超极化激活环核苷酸门控阳离子通道(hyperpolarization activated cyclic nucleotide-gated cation channels,HCN)的表达水平。方法:雄性SD大鼠72只,全部建立CCI模型后随机均分为两组(n=36):脉冲射频组(P组)和对照组(C组)。P组:制模后7 d对其坐骨神经结扎近端行PRF;脉宽20ms,频率500 kHz,脉冲频率2 Hz,温度42℃,持续时间8 min。C组:制模后7 d在相同位置放置射频电极,但无脉冲治疗,持续时间8 min。制模前、制模后7 d、PRF后1、7和14 d测定大鼠疼痛行为学变化。PRF后1、7和14 d,每组各处死12只大鼠,测定DRG中HCN-1和HCN-2的表达水平(其中6只采用免疫组织化学染色法,另外6只采用蛋白质免疫印迹法)。结果:制模后7 d,两组大鼠的热痛阈均显著降低(P<0.01),出现痛觉过敏和痛觉超敏。脉冲射频后,P组的热痛阈逐渐升高,痛觉过敏和痛觉超敏比例逐渐降低,实验结束时两组疼痛行为学差异具有统计学意义(P<0.01)。在实验结束时,P组DRG中HCN-1和HCN-2的表达水平与C组相比显著增加(P<0.01)。结论:HCN通道可能参与PRF的神经调控过程。
Objective: To investigate the pain behavioral changes and the dynamic expression of HCN in the dorsal root ganglion(DRG) of rats with chronic constriction injury of sciatic nerve after pulsed radiofrequency.Methods: Seventy-two male SD rats were randomly divided into two groups: Control group(C group,n = 36) and Pulsed radiofrequency group(P group,n = 36).All rats underwent chronic constriction injury of sciatic nerve.On the 7th postoperative day,in P group,PRF was delivered at 42 ℃ with a pulse duration of 20 ms(500 kHz) and pulse rate of 2 Hz to the proximal sciatic nerve for 8 min;while in C group,PRF needles were placed in the proximal sciatic nerve for 8 min without radiofrequency current.Pain behavioral changes were measured before operation,7th postoperative day,and 1,7,14 days after PRF treatment,respectively.The expression of HCN-1 and HCN-2 were measured on DRG by using immunohistochemistry(n = 6) and Western blot(n = 6) in each group at 1,7,14 days after PRF treatment,respectively.Results: On the 7th postoperative day,paw withdrawal latencies(PWL) of all rats were decreased significantly,mechano-hyperalgesia and mechano-allodynia appeared at the same time.After PRF treatment,PWL of P group were longer than that of C group,and the percentage of mechano-hyperalgesia and mechano-allodynia dropped in P group.At the end of the experiment,pain behavioral changes were significantly different between the two groups(P 0.01),meanwhile the expression of HCN-1 and HCN-2 in the DRG neurons in P group were significantly higher than C group(P 0.01).Conclusion: HCN channels may involve in the neuromodulation of pulsed radiofrequency.
