新型寰椎钩棒及椎板钩联合枢椎椎弓根螺钉内固定的生物力学 被引量:1

A biomechanical evaluation of a self-designed device for C1-C2 fixation

摘要 目的评价A行研制的寰椎后弓环抱钩棒及椎板钩联合枢椎椎弓根螺钉内固定系统的生物力学稳定怵,为临床应用提供实验依据。方法6具新鲜尸体枕颈部标本,采用脊柱三维运动试验机依次对其齿状突切断后(A组)、寰椎椎板钩联合枢椎椎弓根螺钉内固定(B组)、自制寰椎钩棒及椎板钩联合枢椎椎弓根螺钉内固定(C组)、寰椎椎弓根螺钉联合枢椎椎弓根螺钉内固定(D组)等4种状态进行三维运动范围测试;分析比较新型寰枢椎后路内固定系统的稳定性。结果B组后伸平均ROM明显大于C、D组,差异均有统计学意义(P〈0.05);C、D组后伸平均ROM差异无统计学意义(P〉0.05);而B、C、D组间在其他角度ROM(前屈、侧屈、旋转)差异均兀统计学意义(P〉0.05)。结论采用自制寰椎后弓钩棒及椎板钩联合枢椎椎弓根螺钉进行内固定,可为失稳的寰枢关节提供良好的即刻和较长期稳定性。 Objective To evaluate the biomeehanical stability of a self-designed device (CI hook-rod plus C1 laminar hook plus C2 pedicle screws) used for atlantoaxial instability. Methods Cervical specimens were harvested from 6 fresh cadavers to create models of atlantoaxial instability. The specimens were used to test the 3D ranges of motion (ROM) under the following conditions in the following order: odontoid fracture (group A), fixation with C1 laminar hook plus C2 pedicle screws (group B), fixation with C1 hook-rnd plus C1 laminar hook plus C2 pediele screws (group C) and fixalion with CI pedicle screw plus C2 pedicle screws (group D) . The 3D biomechanical stabilities of atlantoaxial segments under 4 different conditions were measured and compared to evaluate the self-designed C1-C2 fixation device. Results In posterior extension, the average ROM in group B was significantly larger than in groups C and D ( P 〈 0. 05) ; there was no significant difference in posterior extension ROM between groups C anti D ( P 〉 0. 05); there were no significant differences regarding ROMs in anterior flexion, lateral flexitm and rotation) between groups B, C and D ( P 〉 0.05) . Conclusion The self-designed C1-C2 fixation device (C1 hook-rod plus C1 laminar hook plus C2 pedicle screws) can provide satisfactoly biomechanical stability for the unstable atlantoaxial joint.
