颈椎棘突骨折三维重建分型及意义的初步探讨 被引量:8

摘要 目的探讨颈椎棘突骨折的CT三维重建分型及临床意义。方法 2010-09-2012-06,对15例颈椎棘突骨折患者行X线、螺旋CT薄层扫描及三维重建检查,建立、评价颈椎棘突骨折的CT三维重建分型及初步探讨其临床意义。结果 X线片检查对颈椎棘突骨折的检出率为60.00%,CT平扫检出率为93.33%,三维重建均检出骨折。结论 X线片对颈椎棘突骨折易造成漏诊或诊断不确切,CT三维重建能直观、精确地显示骨折的立体形态及各解剖结构的空间关系,为建立分型体系提供依据,对选择治疗具有一定的指导作用。 Objective To explore the typing of cervical vertebra spinous process fractures based on tree-dimensional reconstruction by spiral computed tomography(CT) and its clinical significance. Methods 15 cases of cervical vertebra spinous process fracture were examined X-ray plain films, spiral CT scanning and three-dimensional reconstruction from September 2010 to June 2012. The CT three-dimensional reconstruction was evaluated with respect to its clinical typing significance. Results The rate of diagnosis with X-ray plain films was 60%(9/15). The rate of diagnosis with spiral CT scanning was 93.33%(14/15). No diagnosis was missed with CT three-dimensional reconstruction. Conclusion X-ray plain films often cause misdiagnosis or uncertain diagnosis of fractures of cervical vertebra spinous process fracture.But CT three-dimensional reconstruction,which not only displays the stereoscopic shape of the injury region directly and accurately,but also shows spatial relationship of the anatomical details,can provide an accurate basis for the fracture typing and thus guide the treatment.
