摘要
目的探讨采用磁共振(MR,Magnetic resonance)弥散加权成像(DWI,Diffusion-Weighted Imaging)和表观弥散系数(ADC,Apparent diffusion coefficien)诊断椎体压缩骨折良恶性方面的应用价值。方法对25例健康志愿者和89例椎体压缩骨折患者的MRI常规序列及DWI的影像资料统计学分析。结果 1、通过DWI信号强弱判断椎体病变的良恶性方面没有统计学意义p=0.083(p>0.05);2、在b=400s/mm2时,只是感染性病变和恶性肿瘤分别所致的压缩骨折的ADC值比较,无统计学意义(p=0.093,p>0.05),而其余各组之间的ADC差异均具有统计学意义;3、采用常规MRI和采用常规MRI联合DWI时,在诊断椎体病变方面有显著差异,P=0.048(P<0.05)。结论通过DWI成像技术和测量ADC值,可做为鉴别椎体压缩骨折的良恶性一种新的诊断途径和标准;常规MRI序列与DWI技术相结合具有敏感度高、特异度强,在临床诊断中有应用价值。
Objective The objective of our study is to explore the use of Diffusion-Weighted Imaging (DWI) and Apparent Diffusion Coefficient (ADC) of 1.5T MRI tbr the differential diagnosis of benign and malignant vertebral fractures. Materials and Methods Conventional MRI sequences and DWl images of 89 patients with vertebral compression fractures and 35 normal volunteers from August 2009 to December 2010 were collected from our archives. The 94 included in the study sample. The characteristics of diffusion-weighted imaging with different b values of benign and pathological body fractures were compared, and then the analysis of the difference in the apparent diffusion coefficient (ADC) values was carried out. We also tried to explore the value of using conventional MR sequences with DWI in the diagnosis of benign and malignant vertebral compression fracture. Results 1. ADC values between different groups have statistically significant differences (p〈0.05). However, this statistical analyses was not considered significant in the differential diagnosis of benign and malignant vertebral lesions with the signal intensity (p=0.083, p〉0.05). 2. With the b value of 400s/mm2, there was no significant difference in ADC values acquired from compression fractures caused by tumor and infectious diseases (p=0.093, p〉0.05), but the ADC values between the other groups had significant differences statistically. 3. There were also statis- tically significant differences between DWl with the conventional MRI sequences and conventional MRI sequences in diagnosis of benign and malignant vertebral fractures (p=0. 048, P〈0.05). Conclusion The combination of DWI and ADC values provide a new diagnostic approach and diagnostic criteria for the identification of benign and malignant vertebral compression fracture.
