脑钠肽与甲状腺功能亢进性心脏病患者心功能分级和左心室功能的相关性 被引量:2

Correlative study of brain natriuretic peptide with cardiac functional grading and left ventricular function in patients with hyperthyroid heart disease

摘要 目的探讨脑钠肽(BNP)与甲状腺功能亢进性心脏病(HHD)患者心功能分级和左心室功能的相关性。方法 126例甲状腺功能亢进症患者根据是否并发HHD分为HHD组66例和对照组60例,检测2组患者血清BNP、游离型三碘甲状腺原氨酸(FT3)和游离型甲状腺素(FT4)水平,测定2组患者左心室射血分数(LVEF)、二尖瓣口舒张早期血流速度和心房收缩期血流速度比(E/A)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)等心功能指标。结果 HHD组患者血清BNP和FT3水平显著高于对照组(P<0.05),HHD组患者血清FT4水平显著低于对照组(P<0.05);随着HHD患者心功能分级的升高,血清BNP水平逐渐升高(P<0.05),但血清FT3和FT4水平无显著变化(P>0.05)。与对照组相比,HHD组患者LVEF、E/A显著降低(P<0.05),LVESD和LVEDD显著升高(P<0.05)。Pearson相关分析结果显示血清BNP水平与LVEF、E/A呈显著负相关(P<0.05),与LVESD和LVEDD呈显著正相关(P<0.05)。结论检测血清BNP水平有助于HHD的诊断,也可作为评估HHD患者心力衰竭严重程度的指标。 Objective To explore the correlation of brain natriuretic peptide (BNP) with heart function classification and left ventricular function(LVF) in patients with hyperthyroid heart disease(HHD). Methods A total of 126 patients with hyperthyrea were divided into HHD group( n = 66) and control group( n = 60) according to whether HHD was complicated or not, the levels of serum BNP, free triiodothyronine ( FT3 ) , free thyroxine ( FT4 ) , left ventricular ejection fraction ( LVEF ) , the ratio of early-diastolic to late-diastolic peak velocities of the mitral valve orifice(E/A) ,left ventricular end-systolic dimension (LVESD) and left ventricular end-diastolic dimension(LVEDD) were detected in the two groups. Results The levels of serum BNP and FT3 in HHD group were significantly higher than those in control group(P 〈0.05) ,the level of serum FT4 in HHD group was significantly lower than that in control group( P 〈 0.05 ). With the increasing of heart function classification in pa- tients with HHD, serum BNP level increased gradually (P 〈 0.05 ) , but serum FF3 and FT4 levels had no changed significantly ( P 〉 0. 05 ). Compared with control group, the values of LVEF and E/A in HHD group were significantly lower ( P 〈 0.05 ), but the values of LVESD and LVEDD in HHD group were significantly higher( P 〈 0.05 ). Pearson correlation analysis showed that BNP level was negatively related to LVEF and E/A ( P 〈 0. 05 ) , but BNP level was positively related to LVESD and LVEDD ( P 〈 0.05 ). Conclusion BNP level is helpful to the diagnosis of HHD ; BNP can be used as an index to evaluating degree of cardiac failure.
