摘要
目的:探讨青年乳腺癌患者实施保乳手术的可行性及其与非青年乳腺癌保乳手术后的预后差异。方法:回顾性分析西安交通大学医学院第二附属医院2001年1月-2009年12月保乳手术病例,依据诊断时年龄将绝经前患者分为青年组(≤35岁)和对照组(>35岁),结合其临床病理特征分析年龄对乳腺癌保乳术后局部复发、远处转移和总生存率的影响。结果:两组患者临床病理特征基本相同。青年组51例患者,中位随访77月(26-127月),局部复发率为3.9%(2/51),远处转移发生率为17.6%(9/51),生存率为88.2%(45/51)。对照组110例患者,中位随访79月(26-130月),局部复发率为3.6%(4/110),远处转移发生率为13.6%(15/110),生存率为91.8%(101/110);差异均无统计学意义(P>0.05)。结论:相同分期下,青年乳腺癌患者保乳术后的局部复发率、远处转移及生存率并不明显高于非青年组。
Objective:To explore the feasibility of breast-conserving surgery for breast cancer in young women,and the difference of prognosis compared with the non-youth.Methods:A retrospective study was adopted to analyze the breast-conserving surgery cases of our hospital from January 2001 to December 2009.According to the age at diagnosis,premenopausal patients were divided into young group(≤35 years) and control group(35 years),analyzing the impact of age on the local recurrence,distant metastasis and overall survival rate after the breast-conserving surgery.Results:The clinical and pathological features of the two groups were almost the same.51 patients were in the young group,the median follow-up was 77 months(26-127 months),and the local recurrence rate was 3.9%(2/51),the distant metastasis rate was 17.6%(9/51),the overall survival rate was 88.2%(45/51);110 patients were in the control group,the median follow-up was 79 months(26-130months),and the local recurrence rate was 3.6%(4/110),the distant metastasis was 13.6%(15/110),the overall survival rate was 91.8%(101/ 110).The differences of local recurrence,distant metastasis and overall survival rate between two groups had no statistical significance(P0.05 respectively).Conclusion:At the same stage,the local recurrence rate,distant metastasis rate and survival rate of the breast cancer in young women after breast-conserving surgery were not significantly higher than the non-youth.
