摘要 【目的】探讨超声弹性成像与肱动脉反应性充血检测方法评价高血压患者血管早期改变的价值。【方法】采用超声弹性成像技术和肱动脉反应性充血检测方法对64例高血压患者和50例正常人右侧肱动脉血管壁弹性进行评估,其中30例高血压患者在半年降压药物治疗后重新测量相应评价指标。【结果】高血压组肱动脉弹性比值较对照组升高(P〈0.01),内皮依赖性血管舒张功能(FMD%)较对照组降低(P〈0.01),硝酸甘油介导的非内皮依赖性血管舒张功能(NTG%)两组问差异无统计学意义(P〉0.05);半年降压药物治疗后,肱动脉弹性比值较治疗前降低(P〈0.01),FMD%则明显高于治疗前(P〈0.01),NTG%治疗前后差异无统计学意义(P〉0.05)。【结论】超声弹性成像技术与肱动脉反应性充血检测方法均能评价高血压患者血管早期改变,超声弹性成像技术更为方便、快捷。 [Objective] To compare the value of ultrasound elastography vs brachial artery reactive hyperemia measurement in evaluating the early vascular lesions of patients with hypertension. [Methodsl Ultrasound elastography and brachial artery reactive hyperemia measurement were used to assess the elasticity of brachial arterial wall in 64 patients with hypertension and 50 normal controls. Among them, the parameters of 30 hy- pertensive patients after treatment with antihypertensive drug for half a year were measured again. [Resultsl Compared with control group, the strain ratio of brachial arterial wall in hypertension group increased( P 〈 0.01), but endothelium-dependent vasodilatation function(FMD%) decreased( P 〈0.01). There was no significant difference in non-endothelium-dependent vasodilatation function(NTG% ) mediated by nitroglycerin between two groups( P 〉0.05). Compared with before treatment, the strain ratio of brachial artery after the treatment with antihypertensive drugs for half a year decreased( P 〈0.01). FMD% after treatment was obviously higher than that before treatment( P 〈0.01). There was no significant difference in NTG% between before and after treatment( P 〉0.05). [Conclusionl Both ultrasound elastography and brachial artery reactive hyperemia measurement can assess the early vascular lesions of patients with hypertension. Ultrasound elastography technique is more convenient and quick.
