摘要
目的研究世居平原的健康青年男性在3600 m高原习服1个月后乘汽车急进至4400 m高原的心率、血压和血氧饱和度变化规律及与急性高原病之间的关系。方法随机选择127名在3600 m高原习服1个月后乘汽车急进至4400 m高原的健康青年男性作为观察组,填写制式急性高原病流行病学调查表,进行急性高原病路易斯湖评分系统(LLS)评分,有头痛且评分≥3的诊断为急性高原病,测量心率、血压和血氧饱和度。随机选择171名平原健康青年男性作为对照组,填写制式急性高原病流行病学调查表并测量心率、血压和血氧饱和度。结果在高原习服后进入更高海拔,症状主要表现为疲劳虚弱和活动能力下降,急性高原病的发病率为34.65%(44/127),心率、收缩压和舒张压显著高于平原对照组,血氧饱和度显著低于平原对照组(P<0.001)。患病小组的血氧饱和度显著低于未患病小组(P<0.05),但两者的心率、收缩压、舒张压无显著差异(P>0.05)。结论高原习服能够降低进入更高海拔高原后的急性高原病发病率;高原低氧使血液氧饱和度下降,心率增快,血压上升;进入更高海拔高原初期应适当减少活动量以降低氧消耗和改善高原症状,并监测血氧饱和度水平;对于血氧饱和度较低且症状较重者应重点关注并适时采取适当的治疗措施。
Objective To analyze the changes in the heart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP)and arterial oxygen saturation(SaO2)among healthy young men(plain native) entering the 4400-meter plateau by car after a month′s acclimatization on the 3600-meter plateau,and their relationship with the acute mountain sickness(AMS).Methods To tally 127 healthy young men who had entered the 4400-meter plateau by car after a month′s acclimatization at the 3600-meter plateau were randomly selected as the observation group.Questionnaires about symptoms were completed to be scored by the Lake Louise acute mountain sickness scoring system(LLS).The diagnosis of AMS was based on headache and LLS≥3.Another 171 healthy young men living on the plains were randomly selected as the control group.HR,BP and SaO2 were measured in both groups.Results On the 4400-meter plateau,the main symptoms were fatigue or weakness and decline in activity,44 of the 127 participants(34.65%) developed LLSQ AMS.They had higher HR and BP,but lower SaO2 than the control group(P〈0.001).SaO2 in the AMS patients was lower than in non-AMS people(P0.05),however,there were no differences in HR,SBP or DBP(P〈0.05) between the two groups.ConclusionAltitude acclimatization could reduce the incidence of AMS on higher plateaus;plateau hypoxia leads to the decline in SaO2 but increase in HR and BP.We should properly decrease activity to reduce the consumption of oxygen,improve plateau symptoms and monitor the level of oxygen saturation in the initial stage on higher altitude plateaus.Those with low SaO2 and severe symptoms should be taken good care of.
