摘要
为研究高压氧 (HBO)对重型颅脑损伤 (SCI)的治疗效果及机制 ,用经颅超声多普勒 (TCD)观察全脑主要分支动脉收缩期流速、平均流速及搏动指数 (Vs、Vm、Pi)的检查结果及脑电图 (EEG)、脑电地形图(BEAM )的变化情况。结果显示治疗组的临床症状 (GCS)、EEG、GEAM均迅速改善。大脑前动脉 (ACA)、大脑中动脉 (MCA)的Vm分别从 (73 6± 3 1)cm/s和 (85 8± 4 2 )cm/s降至 (6 2 1± 3 9)cm/s和 (79 6± 4 4 )cm/s(P均 <0 0 1) ,相应的Vs及Pi的下降同样有显著性差异。而大脑后动脉、基底动脉的Vm及Vs却显著升高(P均 <0 0 1) ,Pi则仍显著下降 (P均 <0 0 1)。说明HBO能显著改善重型颅脑损伤患者GCS、EEG和BEAM的异常率 ;大脑前、大脑中动脉的血流速度、搏动指数明显下降 ,提示原发损伤部位脑血管痉挛明显缓解 ;大脑后、基底动脉血流速度升高而搏动指数下降则提示非原发损伤区血流阻力减小血流量加大。
In this paper,the efficacy and mechanism of hyperbaric oxygen (HBO) therapy in severe craniocerebral injury (SCI) was studied,56 patients were divided at random into groups treated with BHO(n=31) and control group (n=25).Alterations of brain electrical activity map (BEAM),electroencephalogram (EEG),transcranial Doppler (TCD) were detected before and after HBO therapy.The results showed that BEAM,EEG were obviously improved in the group treated with HBO,as compared with the control group.Anterior cerebral artery (ACA),middle cerebral (MCA) had more difference with posterior cerebral artery(PCA) and basilar artery (BA).It suggests that the HBO could improve the consciousness,BEAM and EEG of SCI patients as a result of remission of the cerebral vascular spasm in MCA & ACA and the cerebral vascular resistence in PCA & BA.This is one of the important mechanisms of HBO therapy to improve prognosis for SCI patients.
