摘要
目的探讨室壁中层缩短率评价2型糖尿病患者左室收缩功能的临床价值。方法 2型糖尿病组患者31例,体检健康者30例为对照组,常规超声心动图测量左室和室间隔相应参数,应用M型超声计算室壁中层缩短率(MFS)、应力相关MFS、左心室径线及圆周收缩末期应力,比较两组间左室结构和收缩功能指标即MFS、左室射血分数(LVEF)和左室短轴缩短率(LVFS),并对糖尿病组MFS与左室心肌质量指数、糖化血红蛋白水平进行相关分析。结果与对照组比较,糖尿病组体质量指数、左室质量、左室心肌质量指数增加(P<0.05),MFS、应力相关MFS减低(P<0.01)。两组间左室收缩末期内径、左室舒张末期内径、舒张期室间隔厚度、舒张期左室后壁厚度、LVEF及LVFS比较,差异均无统计学意义。MFS与左室心肌质量指数呈负相关(r=-0.241,P<0.05),与糖化血红蛋白亦呈负相关(r=-0.414,P<0.05),且这种负相关在校正了年龄、性别、收缩压和体质量指数的影响后仍然存在。结论 MFS是检测2型糖尿病患者早期心肌收缩力下降的敏感指标。
Objective To explore the clinical value of ventricular midwall fractioal shortening (MFS)in evaluationg left ventricular systolic function in patients with type 2 diabetes mellitus (DM). Methods Thirty-one DM patients and 30 healthy volunteers were included in the study. Echocardiography was performed in all participents to detect MFS, S-cMFS, left ventricualr diameter, left ventricular circum ferential end-systolic stress. MFS, left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) were compared between the two groups. The correlations between MFS and the left ventricular mass index (LVMI), glycosylated hemoglobin level were analyzed. Results Compared with control group, DM group had increased body mass index,left ventricular mass and LVMI (P 〈 0.05) , while decreased MFS and S-cMFS (P 〈 0.01 ). There was no difference of left ventricular end-systolic diameter, left ventricular end-diastolic diameter, interventricular septum diastolic thickness and left ventricular posterior wall thickness. MFS was negatively correlated to LVMI (r =-0.241 ,P 〈 0.05)and glycosylated hemoglobin (r=-0.414 ,P 〈 0.05 ). After adjusting the influence of age, gender, systolic blood pressure and body mass index,those negative correlations were still existed. Conclusion MFS is a sensitive parameter for assessing the early ventricular systolic dysfunction in type 2 DM patients.
