摘要
目的:评价托烷司琼联合地塞米松给药对全麻老年患者髋关节置换术后恶心、呕吐的预防效果。方法:90例择期接受气管插管全身麻醉手术的患者,按美国麻醉医师协会(ASA)体格情况分级Ⅰ～Ⅱ级,随机分为3组,A组对照组术毕10min给生理盐水,B组术毕10min给托烷司琼5mg,C组术毕10min给托烷司琼5mg和地塞米松5mg,记录手术时间和麻醉时间,术后患者皮肤瘙痒、嗜睡、呼吸抑制症状及椎体外系反应等不良反应,观察记录术后48h内恶心、呕吐发生例数和程度。结果:3组患者手术时间和麻醉时间,皮肤瘙痒、嗜睡、呼吸抑制症状及椎体外系反应等不良反应比较无统计学意义;3组患者在术后48h内比较,在A组患者中总恶心、呕吐发生例数和呕吐达Ⅲ度发生例数最多有统计学意义,在C组患者中总恶心、呕吐发生例数和呕吐达Ⅲ度发生例数最少有统计学意义。结论:托烷司琼可明显减少髋关节置换老年患者术后恶心呕吐发生,但托烷司琼联合地塞米松给药效果更好。
Objective： To evaluate the preventive effect of tropisetron combined with dexamethasone on nausea and vomiting of aged patients of general anaesthesia after hip replacement. Methods： Ninety patients, accepted general anesthesia surgery by endotracheal intubation, were classified as grade I or II patients in terms of their physical conditions according to ASA. They were randomly divided into 3 groups,who were given sodium Chloride, 5 mg of tropisetron, and 5 mg of tropise-tron combined with 5 mg dexamethasone respectively ten minutes after operations. Then, the time of anaesthsia and surgey, pruritus, drowsiness, respiratory depression symptoms and extrapyrami- dal symptoms were recorded. Also, the number of vomiting and nausea occurred and the degree of the problem as well in the three groups within 48 hrs after operations were observed and recorded. Results： There were no statistical difference for the time of anaesthsia and surgery, and such ad- verse reactions as pruritus, drowsiness, respiratory depression symptoms and extrapyramidal symptoms. Compared with patients in the 3 groups within 48 h after operation, the number of cases with nausea and vomiting and the vomiting degree of grade I]I was the highest in Group A but low- est in Group C, which was of statistical difference. Conclusion： Tropisetron can obviously inhibit the nausea and vomiting of aged patients after hip replacement, but it combined with dexametha- sone will be even more effective.
