火器伤合并海水浸泡兔肢体骨骼肌组织脂质过氧化反应 被引量:7

摘要 目的 观察兔肢体火器伤合并海水浸泡后 ,伤道周围骨骼肌过氧化脂质与抗氧化酶活力的变化 ,为早期救治提供实验依据。方法 用滑膛枪发射 2 5 0mg钢珠 ,致伤兔后肢 ,伤后将致伤兔随机分为两组 :一组为海水浸泡组 (SIG) ,将兔浸泡于粗制海盐配制的人造海水中 3 0min。另一组为单纯致伤组 (SWG) ,伤后不浸泡海水。伤后 3、6、12、2 4h手术取距伤道边缘 0 .5cm (A区 )、1.5cm (B区 )和 2 .5cm(C区 )处肌组织 ,测定ATP含量、丙二醛 (MDA)和超氧化物歧化酶 (SOD)活力。取伤前骨骼肌组织作为对照。结果 伤后SIG兔伤肢骨骼肌MDA含量明显较伤前升高 ,升高幅度与距伤道距离呈反比 (A区 >B区 >C区 ) ,伤后 3、6h升高 ,12h略有下降 ,2 4h再次升高。SWG的MDA变化与SIG相似 ,但升高幅度低于SIG ,两组SOD活力和ATP含量的改变与MDA变化呈相似趋势。结论 肢体火器伤合并海水浸泡加重了伤道周围骨骼肌微循环障碍 ,能量代谢紊乱 ,氧自由基生成增加 ,过氧化脂质反应增强 ,进而引起继发损伤。 Objective To study the changes of lipid peroxidation and antioxidase activity in seawater immersed rabbit limbs after firearm injury. Methods Firearm wounded limbs were established by 250 mg steel ball shot by a gun. The rabbits were randomly divided into group SIG in which the animals had been immersed in artificial seawater for 30 min and group SWG with firearm wound only. The tissues around the wound sites with the distances of 0.5 (area A), 1.5 (area B) and 2.5 cm (area C) were resected respectively 3, 6, 12, 24 h after the firearm injury. ATP content, malondialdehyde (MDA) content and superoxide dismutase (SOD) activity were detected and those parameters in the tissues of contra limb (pre wounded muscle tissues) were determined too as the controls. Results In SIG group, MDA content was much higher in post wounded than in pre wounded group in a reverse ratio with the distances from the wound site (area A>B>C) and it was increased in the 3rd, 6th h post wound, decreased slightly in the 12th h and elevated again in the 24th h. In SWG group, the change of MDA content was similar to that in SIG group, but the increase amplitude was lower than that of SIG group. The changes of ATP content and SOD activity was similar to that of MDA in the 2 groups. Conclusion Firearm wound, accompanied with seawater immersion deteriorates the disturbance of microcirculation and energy metabolism, increase the formation of oxygen free radical and lipid perioxidation reaction and then causes the secondary injuries.
