摘要
目的 探讨治疗难治性脑积水的新方法。方法 采用神经内窥镜的新技术 ,对 3 4例难治性脑积水进行治疗 ,对其中 18例梗阻性脑积水行第三脑室底造瘘 ,对 14例交通性脑积水行脉络丛凝固 ,对 2例伴有单侧室间孔闭塞的交通性脑积水行透明隔造瘘。结果 随访 6~ 2 5个月 ,3 4例中 2 8例 ( 82 % )有效 ,其中 18例梗阻性脑积水全部有效 ,14例交通性脑积水 8例有效 ,6例无效 ,2例伴有单侧室间孔闭塞的交通性脑积水均有效。无严重并发症。结论 采用神经内窥镜的新技术可使极大部分难治性脑积水得到控制 ,应作为难治性脑积水的首选治疗方法。
Objective To explore a new method for refractory hydrocephalus.Methods 34 cases of refractory hydrocephalus were treated by new neuroendoscopic techniques. Among them, 18 cases of occlusive hydrocephalus were treated by endoscopic third ventriculostomy, 14 cases of communicating hydrocephalus by choroid plexus coagulation, and 2 cases of stenosis of monro foramen by ventriculostomy of septum pellucidum. Results After 6~25 months of follow-up, 28 cases improved, 6 cases failed. Conclusion Most refractory hydrocephalus can be controlled by new neuroendoscopic techniques, which may be the method of choice.
