摘要
目的探讨应用MRI T1-map成像技术评价早期髌骨软化症的可行性。方法对26例早期髌骨软化症的患者(男11例,女15例,年龄16~30岁,平均24.1岁)和14名健康志愿者(男6名,女8名,年龄23~28岁,平均25.7岁)均采用Siemens 3.0 T MR超导型扫描仪进行膝关节MRI T1-map成像。分别观察病例组和正常对照组髌骨软骨T1-map彩图的色彩变化,并测得两组的T1值,应用秩和检验对所得T1值的数据进行统计学分析。结果正常对照组髌骨软骨各层面各部位在T1-map上呈均匀的蓝色区域。病例组中有8例髌骨软骨在T1-map出现较多的浅绿色区域,T1值较正常对照组明显增高;16例呈均匀分布的蓝色区域,但T1值增高;2例T1-map呈均匀的蓝色区域,T1值正常。正常对照组髌骨软骨的平均T1值为(1188.1±71.4)ms,病例组髌骨软骨的中位T1值为1272.5 ms(1226.8,1406.6)ms,统计学分析病例组与正常对照组髌骨软骨T1值有显著性差异(P=0.002)。结论 T1-map成像技术可以发现有或者没有形态学改变的髌骨软骨内组织成分的变化,对早期髌骨软化症的诊断和监测具有较好的临床应用价值。
Objective To explore the feasibility of MRI T1-map imaging technique to evaluate early chondromalacia pa-tella of the knee. Methods Clinical confirmed the presence of chondromalacia patella in 26 patients ( 11 men and 15 fe-male, the age ranged from 16 to 30 years, average 24.1 years) and healthy volunteers 14 cases (6 men and 8 female, the age ranged from 23 to 28, average 25.7 years). Knee joint MRI T1-map imaging was performed in both groups using Si-emeds 3.0 T MR superconducting scanner. Observed the color changes in both case group and the normal control group, at the same time, we measured their T~ values. Applied rank sum test for the T1 values data for statistical analysis. Results The blue region representing the lower Tl values in the patella cartilage were uniformed distribution in 14 cases of the nor-mal control group. In the case group, light green area in the patellar cartilage representing the higher T1 values increased in 8 cases, 16 cases's T1 values were obviously higher than the average T1 values of the normal control group, but the color was still evenly distributed, there were no significant changes in T1 values and the color in the 2 cases. The T1 values about the patellar cartilage were (1188.1 ± 71.4) ms in normal control group, while 1272.5ms in case group. There was a signifi-cant difference in T1 values of the patellar cartilage between the case group and the normal control group (P = 0.002). Con-elusion MRI Tl-map imaging technique could found the component changes of the patellar cartilage with or without morpho-logical changes, and has a high clinical value in diagnosis and surveillance in eary chondromalacia patella of knee joint.
