摘要 目的比较七氟醚与异氟醚用于肝脏移植术对肾功能的影响。方法选择原位肝移植患者80例,随机均分异氟醚组(E组)或七氟醚组(S组),两组均采用静-吸复合全身麻醉。麻醉诱导:静脉注射咪达唑仑2~4mg、依托咪酯0.3mg/kg、舒芬太尼0.3~0.5μg/kg、顺阿曲库铵0.1~0.12mg/kg。麻醉维持:舒芬太尼5~10μg(总量1.5~2μg/kg)间断静脉推注,丙泊酚2~4mg·kg-1·h-1、顺阿曲库铵1~2μg·kg-1·h-1静脉持续输注,同时E组吸入1%~2%异氟醚(吸入末浓度1%~1.5%);S组吸入1%~3%七氟醚(吸入末浓度1%~1.5%)。分别于术前(T1)、无肝期前(T2)、无肝期末(T3)、新肝期2h(T4)及术后24h(T5)抽血检测肌酐(Cr)、尿素氮(BUN)、尿酸(UA)、β2-微球蛋白(β2-MG)浓度。结果与T1时比较,T4、T5时两组患者血清Cr、UA和β2-MG浓度明显升高(P<0.05或P<0.01)。各时点两组患者Cr、BUN、UA和β2-MG浓度差异均无统计学意义。结论七氟醚用于肝移植术对患者肾功能影响与异氟醚复合丙泊酚全麻相似。 Objective To compare the effect of sevoflurane and isoflurane on renal function in liver transplantation. Methods Eithty patients undergoing liver transplantation were randomly divided into two groups, received intravenus anesthesia combined with inhalational anesthesia with isoflurane (group E) or sevoflurane (group S) respectively. The renal function was measured by values such as Cr, BUN, UA, β2-MG at pre-operation(T1 ), pre-anhepatic phase (T2), pre- neohepatic phase (T3), 2 hours in neohepatic phase (T4), 24 hours after operation (T5). Results Compared to T1, the value of Cr, BUN, UA and β2-MG at T4 ,Ts increased significantly(P〈0.05) in group E and group S. The variational trend of Cr, Urea, UA and βe-MG between two groups was similar, no statistically significant difference was found. Conclusion The effect of sevoflurane and isoflurane on renal function is similar in liver transplantation.
