摘要 目的:探讨在脓毒症模型中雷公藤内酯醇的抗炎及免疫调节的双重作用效应。方法:60只SD大鼠以盲肠结扎手术制作脓毒症模型,分为假手术组、模型组及雷公藤内酯醇组。采用ELISA检测各组血IL-6、IL-1及TNF-α水平,RT-PCR定量检测各组肺组织中TLR4mRNA及NF-κBp65mRNA的表达,比较各组中肺的湿/干比值。流式细胞仪检测CD4^+/CD8^+比值变化。结果:与假手术组比,模型组中IL-6、IL-1及TNF-a含量增高,肺组织中TLR-4mRNA、NF-κBp65mRNA的表达均增高,CD4^+/CD8^+的含量降低,湿/干肺比值升高,各组差异有统计学意义(P<0.05)。与模型组相比,雷公藤内酯醇组中IL-1、IL-6及TNF-α水平.肺组织中TLR4mRNA及NF-κBp65mRNA的表达,湿/干肺比值均明显降低,CD4^+/CD8^+的含量增高,差异有统计学意义(P<0.05)。结论:在脓毒症中雷公藤内酯醇能抑制炎症级联反应和调节T淋巴细胞亚群CD4^+/CD8^+比值发挥抗炎及调节免疫平衡状态的治疗作用。 Objective:To investigate the anti-inflammatory and immunomodulatory effects of triptolide on sepsis. Methods: Sepsis was induced in 60 SD rats by cecum ligation procedures. The rat models were assigned to sham group ( n = 20), model group ( n = 20, treated with 0.9% saline) and Triptolide treatment group ( n = 20), respectively. Serum interleukin-1 (IL-1), IL-6 and TNF-α were determined by enzyme-linked immunosorbent assay, the pulmonary expression of Toll-like receptor (TLR)-4 mRNA and neural transfer factor- Bp65 mRNA were assessed by quantified reverse transcriptase polymerase chain reaction. Additionally, the ratio of wet to gross weight of the lungs was compared and the CD4 +/CD8 + ratio was obtained by flow cytometer. Results: Compared with sham group, the model group was associated with an increased serum IL-1, IL-6 and TNF-α, elevated pulmonary TLR-4 mRNA and NF-κBp65 mRNA expression and wet/gross weight ratio, as well as reduced the CD4+/CD8+ ratio (all P 〈 0.05 ). The triptolide treatment, when compared with rat models, yielded significantly lower levels of serum IL-1, IL-6 and TNF-α reduced pulmonary TLR-4 mRNA and NF- κBp65 mRNA expression and wet/gross weight ratio, as well as increased CD4+/CD8+ ratio ( all P 〈 0.05 ). Conclusion: Triptoldie may suppress inflammation and modulate immunity by attenuating inflammatory cascades and regulating the CD4+/CD8+ ratio for T lymphocyte subgroups in sepsis.
