摘要
目的通过对颅内脑膜瘤进行术前血管造影检查,评估瘤周引流静脉并进行分型,以指导制定外科治疗策略,提高颅内脑膜瘤的切除效果。方法回顾性分析武汉协和医院2009年~2011年30例颅内脑膜瘤的影像学等临床资料。结果 4例术中因为保护重要引流静脉或静脉窦,未全切。无死亡病例。术后随访28例,时间1~2年,无复发病例。结论脑膜瘤引流静脉的类型是不同的,根据术前血管造影检查,将引流静脉进行分型,继而制定不同的治疗策略,有助于提高切除效果及改善预后。
Objective To guide the microsurgical treatment of introcranial meningiomas and improve surgical outcomes by evaluating the draining veins associated with intracranial meningiomas on the angiogram. Methods Imaging and clinical data of 30 patients who were diagnosed with intracranial meningiomas and received microsurgical treatment between 2009 and 2011 in our hospital were retrospectively analyzed. Results Subtotal removal of meningiomas was performed for protection of the drai- ning vein or sinus in 4 patients out of the 30 patients. Death and severe complication did not occur in all the patients. Twenty- eight patients were followed up for 1 to 2 year after microsurgery,and no patient showed recurrence. Conclusion The draining veins show different patterns. Virtual reality technology can accurately simulate the anatomical feature of the draining veins, which facilitates the planning of individual operations in neurosurgery and can improve surgical outcomes.
