摘要
目的总结应用左西孟旦治疗顽固性心力衰竭患者的护理经验。方法对174例顽固性心力衰竭患者采用左西孟旦治疗,用药期间密切观察临床症状,分析患者的不良反应及恢复情况,并记录护理要点。结果 174例患者中,159例症状好转出院,15例病情恶化死亡。17例患者用药过程中血压下降至90/60mmHg(1mmHg=0.133kPa)以下,其中9例发生持续严重的低血压,给予盐酸多巴胺纠正;15例患者发生心律失常;10例患者出现不同程度的头痛;8例发生低血钾;4例出现恶心呕吐;总体不良反应发生率为31%。结论左西孟旦作为一种新上市的治疗心力衰竭的药物,在临床用药前需严格把握适应症,用药期间严密监护、积极防治不良反应,以保证治疗效果及患者安全。
Objective To summary the clinical observation, adverse effects and nursing experiences of Levosimendan in decompensated heart failure patients. Methods Totally 174 decompensated heart failure patients treated with Levosimendan were observed during the treatment. The clinial outcomes observed include clinical symptoms, adverse effects and the patients＇ recovery. And the key experiences of nursing was recorded. Results Among 174 patients treated with Levosimendan,159 were improved and discharged from hospital,whereas 15 died. Fifty-four patients experienced different adverse events,including hypoten- sion in 17 patients, arrhythmia in 15, headaches in 10, nausea in 4 and hypokalemia in 8. Of 17 patients whose blood pressure was less than 90/60mmHg,nine patients had to receive dopamine administration to maintain blood pressure. All in a11,31% adverse effects were recorded. Conclusion As a new drug for heart failure treatment,we should figure out the indications before using Levosimendan and strictly monitor and prevent the adverse effects of Levosimendan in clinic. These data will be useful for the nursing care of Levosimendan in clinical practice.
