Tumor angiogenesis in meningioma in relation to peritumoral edema, recurrence and prognosis

摘要 目的 探讨肿瘤血管生成与脑膜瘤瘤周水肿、复发和预后的关系 .方法 应用 因子相关抗原抗体免疫组化染色 ,对 5 7例脑膜瘤进行微血管定量研究 ,分析微血管密度(MVD)与脑膜瘤病理类型、瘤周水肿及患者术后复发率、生存期之间关系 .结果 脑膜瘤 5 7例平均 MVD为 34.5 .其中良性、不典型性和恶性脑膜瘤微血管密度分别为 (2 6 .8±9.3) ,(4 1.2± 12 .9)和 (5 2 .7± 14.6 ) ,3种病理类型 MVD均有显著差异 (P<0 .0 1) .中度和重度脑水肿组其瘤组织 MVD(39.4± 11.3和 47.9± 12 .6 )明显高于轻度和无脑水肿组MVD(30 .1± 9.7和 2 5 .2± 8.5 ,P<0 .0 5 ) . MVD≥ 34.5组的 5 a复发率高于 MVD<34.5组 ,且前者 5 a生存率明显差于后者 . AIM To study how angiogenesis in meningioma correlated with peritumoral edema, recurrence and prognosis. METHODS Microvessal count was conducted in 57 cases of meningioma stained immunohistochemicaly with factor Ⅷ related antigen antibody. We analysed the relationship of pathological feature, peritumoral edema, recurrence and prognosis. RESULTS Of all the meningioma, the average microvessel quantity (MVQ) was 34.5. MVD in atypical and malignant meningioma were (41.2±12.9) and (52.7± 14.6 ), far exceeded that in benign meningioma (26.8±9.3, P <0.01). MVD of the no perifocal reaction (25.2±8.5) and slight edema (30.1±9.7) were significantly lower than that of the medium edema (39.4±11.3) and severe edema (47.9± 12.6 , P <0.05). Patients with MVD higher than 34.5 have higher 5 year recurrence rates and lower 5 year survival rates than whose MVD lower than 34.5. CONCLUSION High MVD in meningioma correlates well with tumor invasion, recurrence and poor prognosis.
