慢性肺源性心脏病患者血清缺氧诱导因子-1α和脑利钠肽前体的表达及相关性研究 被引量:3

Expression and related research of hypoxia-indudble factor-1α and NT-proBNP in the serum of patients with pulmonary heart disease

摘要 目的探讨慢性肺源性心脏病(简称肺心病)患者血清缺氧诱导因子-1α(HIF-1α)和脑利钠肽前体(NT-proBNP)的表达及相关性。方法将90例受试者分为肺心病组、慢性阻塞性肺疾病急性发作期(AECOPD)组和对照组,分别采用双抗体夹心酶联免疫吸附法(ELISA)和化学发光法检测受试者血清HIF-lot和NT-proBNP水平;均在未吸氧情况下用血气分析仪测定动脉血氧分压(PaO2);肺心病组及AECOPD组均于入院24小时内行超声心动图检查测定肺动脉收缩压(PASP)。结果肺心病组患者血清HIF-1α和NT-proBNP水平均显著高于AECOPD组;AECOPD。组两项指标显著高于对照组。肺心病组患者HIF-1α、NT-proBNP表达与PASP呈正相关(rHIF-1α=0.91,P〈0.01;rNT-proBNP=0.89,P〈0.01),同时HIF-1α与NT-proBNP的表达也呈正相关(r=0.93,P〈0.01)。结论HIF-1α、NT-proBNP的表达水平能反映缺氧性肺动脉高压的严重程度。 Objective To analysis the expression and correlation of hypoxia-inducible factor-let (HIF-1α) and brain natriuretic peptide (NT-proBNP) in the serum of patients with chronic pulmonary heart disease (cor pulmonale). Methods 90 subjects were divided into 3 groups:pulmonary heart dis- ease group ( n = 30), chronic obstructive pulmonary disease with acute exacerbation (AECOPD) group ( n = 30) and control group ( n = 30). All subjects were tested the content of HIF-la and NT-proBNP in ser- um with a double antibody sandwich enzyme-linked immunosorbent assay (ELISA) and chemilumines- cence detection;they were measured the arterial oxygen pressure (PaO2) in the condition without oxygen with a blood gas analyzer; pulmonary heart disease group and AECOPD group were examined the pulmo- nary artery systolic pressure (PASP) with ultrasound echocardiographic determination when they were ad- mitted to hospital within 24 hours. Results The HIF-1α[ ( 1668.12 ± 679.68) ng/L] and NT-proBNP [ (3381.63 ±1437.68) ng/L] in serum of the pulmonary heart dease were significantly higher than the AECOPD group [ respectively, (924.62 ±294. 64) ng/L and ( 655.27 ± 294.64 ) ng/L ] ; AECOPD group was significantly higher than the healthy control group[respectively, (107.60 ±43.71 ) ng/L and (38.03 ±24.81 ) ng/L]. The expression of HIF-lct,NT-proBNP in pulmonary heart disease group and PASP was being related ( rHIF-1α = 0.909, P 〈 0. 01 ; rNT-proBNP= 0. 892, P 〈 0, 01 ), at the same time the expression of HIF-1α and NT-proBNP was also into being related (r = 0.926, P 〈 0.01 ). Conclusion The expression levels of HIF-1α and NT-proBNP can reflects the severity of the hypoxic pulmonary artery pressure.
