摘要
目的观察分析非动脉炎性前部缺血性视神经病变(NAION)患者的24 h动态血压(ABPM)特征。方法选择63例符合NAION诊断的患者,观察其24 h动态血压监测结果,并与63例性别构成比、年龄相匹配的无NAION患者的动态血压结果相比较。结果①NAION组的24 h平均收缩压、夜间平均收缩压、夜间平均舒张压均显著高于对照组(P<0.05)。②在NAION组中最低舒张压发生在夜间的比率为33.3%,较对照组49.2%减少,但差异无统计学意义,而NAION夜间最低舒张压明显高于对照组,差异有显著性(P<0.05)。③NAION组患者中夜间血压下降率显著低于对照组,差异有显著性(P<0.01)。④NAION组杓型血压的发生率为19.0%,对照组为39.7%,而反杓型血压发生率NAION组为39.7%,对照组为15.9%,差异均有统计学意义(P<0.01)。结论 NAION患者动态血压昼夜节律异常较为普遍,夜间血压(包括动脉压、舒张压)不能正常下降可能为发病因素之一。
Objective To observe the characteristics of changes of 24 - hour ambulatory blood pressure (ABP) of patient with non - arteritic anterior ischemic optic neurepathy (NAION). Methods Sixty - three patients with NAION and 63 age - and gender - matched patients without NAION were enrolled in the study. The parameters of ABP monitoring were recorded and compared. Results The analysis indicated : ① The average 24 h systolic blood pressure (SBP), nighttime SBP and nighttime diastolic blood pressure (DBP) in the patients with NAION were higher than the patients without NAION ( P 〈 0. 05 ). ② The lowest night DBP in NAION group were higher than control group ( P 〈 0. 05 ), but no significant difference was found in the incidence of the lowest DBP in the night between NAION patients (33.3%) and controls (49.2%). ③ The nighttime blood pressure decline rate in NAION group was constantly lower than in control group ( P 〈0.01 ). (4) The rate of dipper blood pressure pattern in NAION group was 19.0% and it was 39.7% in the control group. The rate of reverse - dipper blood pressure pattern in NAION group was 39.7% and it was 15.9% in the control group. The both differences were statistically significant. Conclusion The prevalence of abnormal circadian blood pressure rhythm was quite high in patients with NAION. Improperly declined nocturnal SBP and DBP may be one of the important pathogenetic factors of NAION.
