摘要
为探讨应用可脱性弹簧圈 (MDS或GDC)栓塞脑动脉瘤的效果和经验 ,在神经安定麻醉和全身肝素化条件下 ,采用可脱性弹簧圈经Mag 3F/2F或Tracker 1 0微导管对 86例脑动脉瘤病人进行栓塞治疗 ,其中MDS栓塞 54例 60个动脉瘤 ,GDC栓塞 32例 38个动脉瘤。治疗结果 :成功栓塞 94个动脉瘤 ,其中 1 0 0 %闭塞 64个 ,95%～ 98%闭塞 1 6个 ,90 %～ 95%闭塞 8个 ,80 %～ 90 %闭塞 6个。 4个动脉瘤未栓塞成功。痊愈 83例 ,死亡 3例。 1 6例动脉瘤通过脑血管造影随访 ,随访时间 6～ 1 8个月 ,弹簧圈未完全闭塞者可见瘤颈体残留增大。表明MDS、GDC可有效地闭塞脑动脉瘤 ,创伤性小 ,但操作者应具有一定的经验和技巧。远期效果尚需大宗病例的长期随访观察。
To study the effect and practical experience of intracranial aneurysm embolization with GDC or MDS. Detachable microcoils through 3F/2F or Tracker 10 microcatheter were used for embolism of 86 patients with intracranial aneurysms. Among them ,54 patients with 60 aneurysms were treated by MDS, and 32 patients with 38 aneurysms were treated by GDC.All patients were under neuroleptanalgesia and general heparinization. 94 aneurysms were embolized successfully. Among them, 100% occlusion was achieved in 64 cases, 95%~98% in 16,90%~95% in 8,and 80%~90% in 6.Four cases failed, 83 cases cured,and 3 cases died.16 cases were followed by cerebral angiography, with a follow up period from 6 to 18 months. The patients who were partially embolized showed enlargement of the neck or body in the rest of the aneurysm. Intracranial aneurysm can be effectively embolized with GDC or MDS, the method is safe with minimal injury, but the long term effect of embolization should be followed for a long time.
