摘要
目的:探讨健康人群体表面积、身体质量指数(以下简称"体质指数")对采脉压力及脉图参数的影响。方法:采用DDMX-100型单道脉象仪采集677例健康人群寸口关部脉象信息,对体表面积、体质指数与采脉压力及脉图参数指标进行统计相关分析。结果:男子体表面积、体质指数与采脉压力呈正相关关系,女性体表面积及体质指数与采脉压力无相关关系。男子体表面积与脉图参数左心室快速射血期时值(t1)、左心室收缩期时值(t4)、主波幅度(h1)、重搏波幅度(h5)、脉图总面积(S)、收缩期面积(As)呈正相关关系,男性体质指数与除左心室的舒张期时值(t5)外的其它脉图参数均有相关关系(P<0.05);女性体表面积与脉图参数t1呈正相关关系(P<0.05);女性体质指数与除采脉压力外其它脉图参数均有相关关系(P<0.05)。男、女两性体表面积较大和超重人群的左心室快速射血期和收缩期、主动脉顺应性和外周阻力均高于非肥胖人群。结论:体表面积、体质指数与采脉压力、脉图参数有相关关系,体表面积较大及超重人群的左心室射血功能和动脉血管壁顺应性不及体表面积相对较小、非肥胖人群,在临床进行脉诊时,要考虑到性别、体表面积、体质指数等因素对脉象的影响。
Object: To investigate the influence of surface area, body mass index on pulse pressure, pulse diagram parameters of health people. Methods: We used DDMX-100 single channel pulse instrument on location of guan of 677 health people to acquire pulse information, and statistical analyzed the correlation between the body surface area, body mass index and Pulse pressure, pulse diagram parameters. Results: Male's body surface area, body mass index were positively related to pulse pressure, while fema/e's body surface area, body mass index were not related to pulse pressure. Male's body surface area was positively related to pulse diagram parameters including time of rapid ejection phase (tl) and time of systole phase (t4) of left ventricle, amplitude of main wave (hl), amplitude of repeat wave (h5), square of pulse diagram (S) and area in systole (As). Male's body mass index was related to other pulse diagram parameters except time of diastole phase (t5) of left ventricle (P〈0.05). Female's body surface area was positively related to pulse diagram parameter tl (P〈0.05), female's body mass index was related to other pulse diagram parameters except pulse pressure (P〈0.05). The relatively larger body surface area and mass index left ventricular rapid ejection and systolic period, aortic compliance and resistance in male and female' with were higher than those in non obese people. Conclusions: Body surface area and body mass index are related to pulse pressure and pulse parameters, the left ventricular ejection function and arterial wall compliance in the relatively smaller body surface area and non obesity group are better than those in the larger bady surface area and overweight group. In the clinical pulse diagnosis, we should take into account gender, body surface area, body mass index and other factors effecting on pulse.
